综合性口腔护理干预对ICU呼吸机辅助通气患者呼吸机相关性肺炎的预防效果

Preventive effect of comprehensive oral nursing intervention on ventilatorassociated pneumonia in IcU patients with ventilator-assisted ventilation

目的 探讨综合性口腔护理干预对ICU呼吸机辅助通气患者呼吸机相关性肺炎(ventilator associated pneumonia,VAP)的预防效果.方法 将2020年4月-2021年4月山东省单县海吉亚医院收治的78例ICU呼吸机辅助通气患者随机分为对照组(n=39)和研究组(n=39),对照组采取常规口腔护理,研究组采取综合性口腔护理,比较两组VAP发生率、口腔卫生状况、口腔并发症情况、机械通气时间及ICU入住时间.结果 研究组VAP发生率、口腔并发症发生率均低于对照组(P<0.05);护理后,研究组口腔软垢指数、口腔清洁度、牙菌斑指数等口腔卫生状况均优于对照组(P<0.05);研究组机械通气时间及ICU入住时间均短于对照组(P<0.05).结论 综合性口腔护理干预有助于改善ICU呼吸机辅助通气患者口腔卫生状况,减少口腔并发症,进而有效预防VAP,并减少患者机械通气时间及ICU入住时间,值得推广.

Objective To investigate the preventive effect of comprehensive oral nursing intervention on ventilator associated pneumonia(VAP)in ICU patients with ventilator-assisted ventilation.Methods From April 2020 to April 2021,78 cases of ICU patients with ventilator assisted ventilation were treated in our hospital and were randomly divided into control group(n=39)and study group(n=39).The control group was given routine oral care,while the study group was given comprehensive oral care.The incidence of VAP,oral hygiene,oral complications,duration of mechanical ventilation and ICU stay were compared between the two groups.Results The incidence of VAP and oral complications in the study group were lower than those in the control group(P<0.05);the oral soft scale index,oral cleanliness and dental plaque index of the study group were better than those of the control group(P<0.05);the duration of mechanical ventilation and ICU stay in the study group were shorter than those in the control group(P<0.05).Conclusion Comprehensive oral nursing intervention can improve the oral health status of ICU patients with ventilator assisted ventilation,reduce oral complications,effectively prevent VAP,and reduce the time of mechanical ventilation and ICU stay,which is worthy of promotion.
