克拉霉素联合泼尼松对CT显示肺部磨玻璃结节患者结节直径、密度的影响

Effect of clarithromycin combined with prednisone on diameter and density of ground glass nodules in patients with CT

目的探究克拉霉素联合泼尼松对CT显示肺部磨玻璃结节患者结节直径、密度的影响.方法选取南华大学附属长沙中心医院2021年1月-12月期间收治的98例CT显示肺部磨玻璃结节患者,以随机数字表法分为A组(49例)、B组(49例),分别应用泼尼松治疗、泼尼松联合克拉霉素治疗,比较两组治疗效果、治疗前后肺部CT影像(结节直径、结节密度)、康复时间(退热时间、咳嗽消失时间、肺啰音消失时间)及治疗不良反应发生率.结果B组治疗有效率为95.92%,较A组的75.59%高,差异有统计学意义(P<0.05);治疗前B组磨玻璃结节直径、结节密度水平与A组相近,差异无统计学意义(P>0.05);治疗完成时、治疗完成后1个月时B组磨玻璃影结节直径、密度水平均较A组低,差异有统计学意义(P<0.05);B组退热时间、咳嗽消失时间、肺啰音消失时间均较A组短,差异有统计学意义(P<0.05);B组不良反应发生率为8.16%,与A组的6.12%相近,差异无统计学意义(P>0.05).结论对CT显示肺部磨玻璃结节患者应用克拉霉素联合泼尼松治疗,可提升治疗效果,改善其CT显示肺部磨玻璃结节直径及密度,缩短其康复时间,且不增加治疗不良反应.

Objective To explore the effect of clarithromycin combined with prednisone on the diameter and density of pulmonary ground-glass nodules shown by CT.Methods From January 2021 to December 2021,98 patients with ground-glass nodules in the lung were selected and divided into group A(n=49)and group B(n=49)by random numher table method.They were treated with prednisone and prednisone combined with clarithromycin,respectively.The treatment effect,lung CT images(nodule diameter,nodule density),recovery time(antipyretic time,cough disappearance time,lung rales disappearance time)and adverse reactions were compared between the two groups before and after treatment.Resul ts The effective rate of group B was 95.92%,higher than that of group A 75.59%,and the difference was statistically significant(P<0.05).Before treatment,the diameter and density of ground glass nodules in group B were similar to those in group A,and the differences were not statistically significant(P>0.05).At the completion of treatment and one month after the completion of treatment,the diameter and density of ground glass nodules in group B were lower than those in group A,and the differences were statistically significant(P<0.05).The antipyretic time,cough disappearance time and lung rales disappearance time in group B were shorter than those in group A,and the differences were statistically significant(P<0.05).The incidence of adverse reactions in group B was 8.16%,which was similar to 6.12% in group A,and the difference was not statistically significant(P>0.05).Conclusion Clarithromycin combined with prednisone can improve the therapeutic effect,improve the diameter and density of pulmonary ground-glass nodules shown by CT,shorten the recovery time,and do not increase the adverse reactions of treatment.
