胸椎旁神经阻滞对胸腔镜肺癌根治术病人镇痛效果及不良反应情况的影响

Effects of thoracic paravertebral nerve block on analgesic effect and adverse reactions in patients undergoing thoracoscopic radical resection of lungcancer

目的分析胸椎旁神经阻滞在胸腔镜肺癌根治术中对患者镇痛效果与不良反应的影响.方法随机选取94例在湖南省郴州市第三人民医院接受治疗的胸腔镜肺癌根治术患者,将47例患者纳为研究组在麻醉诱导前接受胸椎旁神经阻滞,将47例患者纳为对照组在麻醉诱导前不接受任何处理措施.对比患者的疼痛评价、术后静脉自控泵按压与背景输注情况以及不良反应情况.结果研究组术后2h、24h静息状态与活动状态下的疼痛评分均低于对照组,术后24h、48h的静脉自控泵按压次数与背景输注量均少于对照组,术后不良反应发生率低于对照组(P<0.05).结论在胸腔镜肺癌根治术中对患者实施胸椎旁神经阻滞,可有效提升患者手术过程中的镇痛效果,且安全性表现较好,患者不良反应较少.

Objective To analyze the analgesic effect and adverse reactions of thoracic paravertebral nerve block in patients undergoing thoracoscopic radical resection of lung cancer.Methods A total of 94 patients undergoing thoracoscopic radical resection of lung cancer in our hospital were randomly selected.47 patients were included in the study group who received thoracic paravertebral nerve block before induction of anesthesia,and 47 patients were included in the control group who did not receive any treatment before induction of anesthesia.The evaluation of pain,postoperative automatic intravenous pumping and background infusion,and adverse reactions were compared.Results The pain scores in the resting state and active state at 2h and 24h after operation in the study group were lower than those in the control group,the times of automatic intravenous pump pressing and background infusion volume at 24h and 48h after operation were less than those in the control group,and the incidence of postoperative adverse reactions was lower than that in the control group(P<0.05).Conc lusion Thoracic paravertebral nerve block in thoracoscopic radical resection of lung cancer can effectively improve the analgesic effect of patients during the operation,with good safety performance and fewer adverse reactions.
