胎膜早破患者发生排尿困难的相关危险因素分析

Analysis of risk factors for dysuria in patients with premature rupture of membranes

目的分析胎膜早破(PROM)患者发生排尿困难的相关危险因素.方法选取惠州市第一妇幼保健院2020年3月-2021年10月收治的117例PROM患者,评估患者排尿困难发生情况;调查患者相关资料,将可能的危险因素纳入,回归分析PROM患者发生排尿困难的相关危险因素.结果117例PROM患者中有29例发生排尿困难,发生率为24.79%(29/117);发生排尿困难的PROM患者分娩经历、卧床时间、尿路感染、合并妊娠期糖尿病、排尿姿势与未发生患者对比,差异有统计学意义(P<0.05);经Logistics回归分析检验,结果显示,初产、卧床时间≥5h、尿路感染、合并妊娠期糖尿病、仰卧位排尿姿势是PROM患者排尿困难的危险因素(OR>1,P<0.05).结论PROM患者发生排尿困难可能与初产、卧床时间≥5h、尿路感染、合并妊娠期糖尿病、仰卧位排尿姿势有关,未来可据此提出针对性护理方案,以预防排尿困难发生.

Objective To analyze the risk factors of dysuria in patients with premature rupture of membranes(PROM).Methods A total of 117 PROM patients admitted to Huizhou First Maternal and Child Health Hospital from March 2020 to October 2021 were selected.The relevant data of patients were investigated and the possible risk factors were included to regression analyze the risk factors of dysuria in PROM patients.ResuIts Among 117 PROM patients,29 had dysuria(24.79%,29/117).There were significant differences in delivery experience,hed rest time,urinary tract infection,gestational diabetes mellitus,and urination posture between PROM patients with dysuria and those without(P<0.05).Logistics regression analysis showed that primary labor,bed rest time≥5 hours,urinary tract infection,gestational diabetes mellitus and supine position were risk factors for dysuria in PROM patients(OR>1,P<0.05).Conclusions Dysuria in PROM patients may be related to primary labor,hed rest time≥5 hours,urinary tract infection,gestational diabetes melltus,supine position.In the future,targeted nursing programs can be proposed to prevent dysuria.
