互联网+全科团队模式在老年慢性病健康管理的应用现状

The Current Application of Internet Plus General Practitioner Team Model on Health Management of Geriatrics Chronic Diseases

随着经济发展及老年人慢性病发病率的升高,老年人对医疗服务及健康需求也越来越高。随着“互联网+”在疾病管理中的广泛应用,全科团队在慢性病管理中重要性的增加,“互联网+全科团队”的服务模式也应运而生。本文对“互联网+全科团队”模式的定义、政策、在老年慢性病中的应用现状、面临的主要问题及可能对策作一综述,以期提高人们对这一健康管理服务模式的认知,并推动其积极发展。

The elderly have an increasing demand for medical services and health with the economic development and the increasing incidence of chronic diseases.Internet plus is more and more widely employed in disease management and general practitioner team has been paid more and more attention in the management of chronic diseases.Therefore,the service model of internet plus general practitioner team has emerged.The definition,policies,application status,main problems and possible countermeasures of the internet plus general practitioner team model in geriatrics chronic diseases are reviewed,in order to improve people’s awareness of this health management service model and promote its positive development.
