血清miRNA水平与急性缺血性脑卒中患者病情程度和炎症反应的相关性研究

Correlation of serum levels of miRNAs with the disease severity and inflammatory response in patients with acute ischemic stroke

摘要 目的探究急性缺血性脑卒中(AIS)患者外周血微小RNA(miRNA)-30a(miR-30a)、miR-122、miR-126、miR-221、miR-409水平与病情严重程度和炎症反应的相关性。方法选择2020年2月至2022年2月浙江新安国际医院收治的90例AIS患者为观察组,以同期在本院健康体检正常者110名作为对照组。检测两组血清miRNA和血清炎症因子水平,分析AIS患者血清miRNA水平与其梗死体积、美国国立卫生研究院卒中量表(NIHSS)评分和血清炎症因子的相关性。结果与对照组相比,观察组患者血清miR-30a、miR-122、miR-126、miR-221和CRP、TNF-α、IL-12、IL-35水平显著升高,miR-409水平显著降低,差异均有统计学意义(均P<0.05)。与小体积梗死组相比,大体积梗死组患者血清miR-30a、miR-122、miR-126、miR-221及CRP、TNF-α、IL-12、IL-35水平更高,miR-409水平更低,差异均有统计学意义(均P<0.05)。与轻度卒中组相比,中重度卒中组患者血清miR-30a、miR-122、miR-126、miR-221及CRP、TNF-α、IL-12、IL-35水平更高,miR-409水平更低,差异均有统计学意义(均P<0.05)。Pearson相关性分析表明,AIS患者血清miR-30a、miR-122、miR-126、miR-221水平与梗死体积、NIHSS评分及CRP、TNF-α、IL-12、IL-35均呈正相关(均P<0.05);而miR-409水平与梗死体积、NIHSS评分及CRP、TNF-α、IL-12、IL-35水平均呈负相关(均P<0.05)。结论AIS患者血清miR-30a、miR-122、miR-126、miR-221水平升高,miR-409水平降低。此5种miRNA水平与患者病情严重程度和炎症反应程度相关,对其开展检测将有助于AIS患者病情的评估。 Objective To explore the correlation of serum levels of microRNAs with the disease severity and inflammatory response in patients with acute ischemic stroke(AIS).Methods Ninety patients with AIS treated in Zhejiang Xin'an International Hospital from February 2020 to February 2022 were enrolled in the study,and 110 healthy subjects receiving physical examination during the same period were selected as the control group.The serum miR-30,miR-122,miR-126,miR-221,miR-409 and inflammatory factors were detected in two groups,and the correlation of serum miRNA level with infarction volume,national institutes of health stroke scale(NIHSS)score and serum inflammatory factors in AIS patients was analyzed.Results Compared with the control group,the serum levels of miR-30a,miR-122,miR-126,miR-221,CRP,TNF-α,IL-12 and IL-35 in AIS group were significantly increased,while miR-409 was significantly decreased(all P<0.05).Compared with the small volume infarction group,the serum levels of miR-30a,miR-122,miR-126,miR-22,CRP,TNF-α,IL-12,IL-35 in the large volume infarction group were significantly increased;and miR-409 was was significantly decreased(all P<0.05).Compared with the mild stroke group,the serum levels of miR-30a,miR-122,miR-126,miR-221,CRP,TNF-α,IL-12 and IL-35 in the moderate and severe stroke group were significantly increased,while miR-409 was significantly decreased(all P<0.05).Pearson correlation analysis showed that the levels of serum miR-30a,miR-122,miR-126,miR-221 in the AIS group were positively correlated with infarct volume,NIHSS score,serum CRP,TNF-α,IL-12 and IL-35 levels(all P<0.05);miR-409 was negatively correlated with infarct volume,NIHSS score,serum CRP,TNF-α,IL-12 and IL-35 levels(all P<0.05).Conclusion The serum levels of miR-30a,miR-122,miR-126 and miR-221 are increased,while miR-409 decreased,which are related to the disease severity and the inflammatory reaction of AIS patients.
