腹腔镜胆囊切除术对胆结石患者术后应激反应及胃肠功能恢复的影响

Effect of laparoscopic cholecystectomy for the patients with gallstones on postoperative stress response and recovery of gastrointestinal function

目的探讨实施腹腔镜胆囊切除术治疗胆结石患者对应激反应指标的影响。方法选择2020年1月至2022年3月句容市人民医院收诊治疗的胆结石患者78例,根据随机数字表法分为对照组(39例)与观察组(39例)。对照组患者采取小切口手术治疗,观察组患者实施腹腔镜胆囊切除术治疗,监测对比两组患者手术前后应激指标变化,记录胃肠功能恢复用时情况,对比胃肠激素水平变化。结果观察组患者术后内皮素、皮质醇水平均明显低于对照组,血清超氧化物歧化酶高于对照组(P<0.05)。观察组患者术后恢复时间短于对照组,胃动素、胃泌素监测浓度均高于对照组,差异均有显著性(P<0.05)。结论实施腹腔镜胆囊切除术治疗胆结石患者,能够有效降低患者术后应激反应,同时促进胃肠功能恢复。

Objective To investigate the effect of laparoscopic cholecystectomy on stress response indicators in patients with gallstones.Method 78 patients with gallstones who were treated in Jurong People's Hospital from January 2020 to March 2022 were divided into control group(39 cases)and observation group(39 cases)according to random number table.The changes of stress indexes were observed before and after surgery in the two groups.The recovery time of gastrointestinal function and comparing the changes of gastrointestinal hormone levels were also observed.Result The levels of endothelin and cortisol in the observation group were significantly lower than those in the control group,and the levels of serum superoxide dismutase were higher than those in the control group(P<0.05).The postoperative recovery time of patients in the observation group was shorter than that in the control group,and the monitoring concentrations of motilin and gastrin were higher than those in the control group,with significant differences(P<0.05).Conclusion Laparoscopic cholecystectomy for patients with gallstones can effectively reduce postoperative stress response and promote the recovery of gastrointestinal function.
