经尿道等离子前列腺剜除术治疗良性前列腺增生的效果观察

Effect of Transurethral Plasma Kinetic Enucleation of Prostate on Benign Prostatic Hyperplasia

目的:评估经尿道等离子前列腺剜除术(TUKEP)应用在良性前列腺增生(BPH)患者中的效果。方法:选择2021年3月—2022年10月梧州市工人医院收治的80例BPH患者,参照随机数表法划分为对照组[行经尿道前列腺双极等离子电切术(TUPKP)]、观察组(行TUKEP),各40例。评价两组的围手术期指标、尿流动力学、生活质量量表(QOL)、国际前列腺症状量表(IPSS)、国际勃起功能指数量表(IIEF-5)、术中并发症及近期并发症发生率。结果:观察组各项围手术期指标均优于对照组,差异有统计学意义(P<0.05)。术前,两组的尿流动力学、QOL、IPSS、IIEF-5比较,差异无统计学意义(P>0.05);术后,观察组尿流动力学指标、IIEF-5评分优于对照组,差异有统计学意义(P<0.05),而两组间QOL、IPSS评分比较,差异无统计学意义(P>0.05)。两组的术中并发症发生率比较,差异无统计学意义(P>0.05),但观察组近期并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:TUPKP、TUKEP均能有效治疗BPH,可以减轻症状,提高生活质量;TUKEP在改善围手术期指标及尿流动力学、减少并发症方面更具优势,且不易影响性功能。

Objective:To evaluate the effect of transurethral plasma kinetic enucleation of prostate(TUKEP)transurethral plasma enucleation in patients with benign prostatic hyperplasia(BPH).Method:A total of 80 patients with BPH admitted to Wuzhou Workers'Hospital from March 2021 to October 2022 were selected and divided into control group[underwent transurethral bipolar plasma kinetic prostatectomy(TUPKP)]and observation group(underwent TUKEP)according to random number table method,40 cases in each group.The perioperative indexes,urodynamics,quality of life scale(QOL),international prostate symptom scale(IPSS),international erectile function index scale(IIEF-5),intraoperative complications and recent complications of the two groups were evaluated.Result:The perioperative indexes of the observation group were better than those of the control group,and the differences were statistically significant(P<0.05).Before operation,there were no significant differences in urodynamics,QOL,IPSS and IIEF-5 between the two groups(P>0.05).After operation,the urodynamics indexes and IIEF-5 scores in the observation group were better than those in the control group,the differences were statistically significant(P<0.05).However,there were no statistically significant differences in QOL and IPSS scores between the two groups(P>0.05).There was no significant difference in the incidence of intraoperative complications between the two groups(P>0.05),but the incidence of recent complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:TUPKP and TUKEP can effectively treat BPH,alleviate symptoms and improve quality of life;while the TUKEP has more advantages in improving perioperative indicators and urodynamics,reducing complications,and is not easy to affect sexual function.
