CT血管造影三维重建技术对肺部占位性病变的定性诊断价值

Value of Three-dimensional Reconstruction of CT Angiography in the Qualitative Diagnosis of Pulmonary Space Occupying Lesions

摘要 目的:探讨CT血管造影三维重建技术对肺部占位性病变的定性诊断价值。方法:回顾性分析2019年1月—2021年12月灌云县人民医院收治的72例肺部占位性病变患者的一般资料。所有患者均行CT血管造影三维重建技术检查,接受支气管纤维镜组织活检或接受手术治疗患者术中取组织进行病理检查。以病理结果为金标准,分析CT血管造影三维重建技术参数对肺部占位性恶性病变检查的诊断价值。结果:72例肺部占位性病变患者中,40例为恶性病变,占55.56%(小细胞肺癌10例,腺癌23例,鳞癌5例,其他2例);32例为良性病变,占44.44%(肺炎20例,肺结核7例,肺硬化性血管瘤5例)。CT血管造影三维重建技术检查肺部占位性病变的体积为(636.74±1.32)mm^(3)、曲面成像(6.62±0.52)分、最大密度投影(6.78±0.53)分、容积再现(6.58±0.46)分。CT血管造影三维重建技术参数对肺部占位恶性性病的诊断敏感度为87.50%(35/40)、特异度93.75%(30/32)、准确度为90.28%(65/72)。结论:CT血管造影三维重建技术通过检测体积、直径、曲面成像、最大密度投影、容积再现等参数,对肺部占位性病变的定性诊断价值较高。 Objective:To evaluate the value of three-dimensional reconstruction of CT angiography in the qualitative diagnosis of pulmonary space occupying lesions.Method:The general data of 72 patients with pulmonary space occupying lesions admitted to Guanyun County People’s Hospital from January 2019 to December 2021 were analyzed retrospectively.All patients were examined with CT angiography three-dimensional reconstruction technology,and underwent bronchofibroscope tissue biopsy or surgical treatment.Based on the pathological results as the gold standard,the diagnostic value of three-dimensional reconstruction parameters of CT angiography in the examination of pulmonary space-occupying malignant lesions was analyzed.Result:Among the 72 patients with lung space-occupying lesions,40 cases were malignant lesions,accounting for 55.56%(10 cases of small cell lung cancer,23 cases of adenocarcinoma,5 cases of squamous cell carcinoma,and 2 other cases);32 cases were benign lesions,accounting for 44.44%(20cases of pneumonia,7 cases of pulmonary tuberculosis,5 cases of pulmonary sclerosing hemangioma).The volume of pulmonary space occupying lesions examined by three-dimensional reconstruction technique of CT angiography was(636.74±1.32)mm^(3),curved surface imaging(6.62±0.52)points,maximum density projection(6.78±0.53)points,and volume rendering(6.58±0.46)points.The sensitivity,specificity and accuracy of 3D reconstruction parameters of CT angiography in the diagnosis of pulmonary space-occupying malignancies were 87.50%(35/40),93.75%(30/32)and 90.28%(65/72)respectively.Conclusion:In the qualitative diagnosis of pulmonary space occupying lesions by three-dimensional reconstruction of CT angiography,the maximum diameter of the lesion is small,and the volume,curved surface imaging,maximum density projection and volume reproduction are obvious.It has high value in the qualitative diagnosis of pulmonary space occupying lesions.
