血清高迁移率族蛋白B1、C反应蛋白、白三烯B4以及降钙素原对新生儿重度肺炎的诊断价值 被引量:3

摘要 目的探究血清高迁移率族蛋白B1(HMGB1)、C反应蛋白(CRP)、白三烯B4(LTB4)以及降钙素原(PCT)对新生儿重度肺炎的诊断价值。方法选择2020年1月至2022年1月于惠东县人民医院接受治疗的90例患肺炎的新生儿作为肺炎组,以患儿病情严重程度分为中轻度肺炎组(50例)和重度肺炎组(40例),再选取30例健康新生儿作为对照组,其中重度肺炎组分为危重组(8例)、一般危重组(12例)和非危重组(20例)。比较各组HMGB1、CRP、LTB4和PCT水平,以及分析上述指标平均肺炎严重程度的价值。结果肺炎组患儿HMGB1、CRP、LTB4和PCT水平高于对照组,差异有统计学意义(P<0.05);重度肺炎组患儿HMGB1、CRP、LTB4和PCT水平高于中轻度肺炎组,一般危重组、危重组HMGB1、CRP、LTB4和PCT水平均高于非危重组;且危重组HMGB1、CRP、LTB4和PCT水平均高于一般危重组,差异有统计学意义(P<0.05);HMGB1、CRP、LTB4、PCT、HMGB1+CRP、LTB4+PCT+HMGB1、LTB4+PCT+HMGB1+CRP诊断重度肺炎误诊率比较,差异有统计学意义(P<0.05);HMGB1、CRP、LTB4、PCT、HMGB1+CRP、LTB4+PCT+HMGB1、LTB4+PCT+HMGB1+CRP诊断重度肺炎的特异度、敏感度与准确度比较,差异有统计学意义(P<0.05)。结论HMGB1、CRP、LTB4和PCT可以有效的诊断新生儿肺炎和反映肺炎严重程度,可以作为诊断新生儿肺炎和评估病情的指标。 Objective To explore the diagnostic value of serum high-mobility group protein B1(HMGB1),C-reactive protein(CRP),leukotriene B4(LTB4),and procalcitonin(PCT)for severe neonatal pneumonia.Methods A total of 90 neonates with pneumonia who were treated in Huidong County People's Hospital from January 2020 to January 2022 were selected as the pneumonia group,and the children were divided into ordinary pneumonia group(50 cases)and severe pneumonia group(40 cases)according to the severity of the disease and 30 healthy newborns were selected as the control group.The severe pneumonia group was divided into critical group(8 cases),general critical group(12 cases)and non-critical group(20 cases).The levels of HMGB1,CRP,LTB4 and PCT in each group were compared,and the value of the average pneumonia severity of the above indicators was analyzed.Results The levels of HMGB1,CRP,LTB4 and PCT in the pneumonia group were higher than those in the control group,and the differences were statistically significant(P<0.05).The misdiagnosis rates of HMGB1,CRP,LTB4,PCT,HMGB1+CRP,LTB4+PCT+HMGB1,LTB4+PCT+HMGB1+CRP were different in the diagnosis of severe pneumonia,and the differences were statistically significant(P<0.05);HMGB1,CRP,LTB4,PCT,HMGB1+CRP,LTB4+PCT+HMGB1,LTB4+PCT+HMGB1+CRP were different in specificity,sensitivity and accuracy in the diagnosis of severe pneumonia,and the differences were statistically significant(P<0.05).Conclusion HMGB1,CRP,LTB4 and PCT can effectively diagnose neonatal pneumonia and reflect the severity of pneumonia,which can be used as indicators to diagnose neonatal pneumonia and evaluate the condition.
