心导管室护士对胸痛护理培训需求的调查及分析

Investigation and analysis of nursing training needs of cardiac catheterization unit nurses for chest pain

目的了解心导管室护士胸痛护理培训内容的需求情况,为制订科学合理的培训计划提供参考依据。方法采用便利抽样法,于2021年3月至4月对国内27个省、自治区、直辖市(贵州、山西、陕西、上海、广西、河北、江西、重庆、新疆、四川、福建、湖北、天津、黑龙江、江苏、安徽、湖南、广东、海南、北京、浙江、辽宁、河南、内蒙古、甘肃、山东、云南)的胸痛中心心导管室护士进行网络问卷调查,采用自制的胸痛护理知识培训需求调查表对心导管室护士进行一般资料、课程内容需求和期望的培训方式进行调查。结果最终收集365份有效问卷,心导管室护士胸痛护理培训最需求的内容分别为胸痛中心的救治流程设置(80.3%)、溶栓治疗的标准操作规程(69.9%)、急性心肌梗死中常见心律失常的心电图诊断(69.3%)、胸痛中心各项管理制度(63.6%)和胸痛中心的时间节点管理要求(63.00%)。不同学历护士在心肺复苏技能、院内发生急性冠状动脉综合征或心脏骤停处理流程方面的需求率差异有统计学意义(P<0.05);不同职称护士在急性主动脉夹层的临床表现、诊断及治疗指南、急性心肌梗死中常见心律失常的心电图诊断、心肺复苏技能、院内发生急性冠状动脉综合征或心脏骤停的处理流程、初级心肺复苏技能此项培训方面的需求率差异有统计学意义(P<0.05);不同行政职务护士在胸痛中心各项管理制度、数据填报与管理、急性心肌梗死中常见心律失常的心电图诊断方面的需求率差异有统计学意义(P<0.05)。心导管室护士对培训方式需求最高的前三位分别是网络视频学习(76.44%)、案例教学(63.56%)和情景模拟训练(58.08%)。结论心导管室护士对胸痛知识的培训需求在学历、职称、行政职务方面存在差异,应针对护士的学历、职称、行政职务等制订个性化培训内容,提升护士岗位胜任力。

Objective To understand the needs of chest pain nursing training for nurses in cardiac catheterization unit,and to provide reference basis for formulating scientific and reasonable training plan.Methods A convenient sampling method was used from March to April 2021 to conduct an online questionnaire survey of nurses in cardiac catheterization unit of chest pain centers in 27 provinces,autonomous regions,and municipalities(Guizhou,Shanxi,Shaanxi,Shanghai,Guangxi,Hebei,Jiangxi,Chongqing,Xinjiang,Sichuan,Fujian,Hubei,Tianjin,Heilongjiang,Jiangsu,Anhui,Hunan,Guangdong,Hainan,Beijing,Zhejiang,Liaoning,Henan,Inner Mongolia,Gansu,Shandong,Yunnan),The self-created questionnaire for training needs of chest pain nursing knowledge was used to investigate general information,course content needs,and expected training methods of nurses in cardiac catheterization unit.Results Finally,365 valid questionnaires were collected.The most important contents of chest pain nursing training for nurses in the cardiac catheterization unit were the establishment of a rescue process in the chest pain center(80.3%),standard operating procedures for thrombolytic therapy(69.9%),ECG diagnosis of acute myocardial infarction and common arrhythmia(69.3%),various management systems in the chest pain center(63.6%),and time node management requirements in the chest pain center(63.00%).There were statistically significant differences in the demand rate of nurses with different educational backgrounds in terms of cardiopulmonary resuscitation skills and the process of dealing with acute coronary syndrome or cardiac arrest in hospital(P<0.05).There were statistically significant differences in the demand rate of nurses with different professional titles in the training of clinical manifestations,diagnosis and treatment guidelines of acute aortic dissection,ECG diagnosis of common arrhythmia in acute myocardial infarction,cardiopulmonary resuscitation skills,treatment process of acute coronary syndrome or cardiac arrest in hospital,and primary cardiopulmonary resuscitation skills(P<0.05).There were statistically significant differences in the demand rate of nurses with different administrative positions in various management systems,data filling and management,ECG diagnosis of common arrhythmia in acute myocardial infarction in the chest pain center(P<0.05).The top three training methods requested by nurses in the cardiac catheterization unit were online video learning(76.44%),case teaching(63.56%)and scenario simulation training(58.08%).Conclusion There are differences in the training needs of cardiac catheterization unit nurses for chest pain knowledge in terms of educational background,professional title and administrative position.Personalized training content should be formulated according to the educational background,professional title and administrative position of nurses to improve their competency.
