左卡尼汀联合胰激肽原酶治疗弱精子症伴有精索静脉曲张术后患者的疗效观察

Therapeutic Effect of L-carnitine Combined with Pancreatic Kininogenase on Patients with Asthenospermia Complicated with Varicocele after Microsurgical Varicocelectomy

目的 观察左卡尼汀口服液联合胰激肽原酶肠溶片治疗弱精子症伴有精索静脉曲张患者显微结扎术后的疗效。方法 回顾性分析79例精索静脉曲张伴有弱精子症的患者,随机分为对照组39例和治疗组40例,所有患者均行精索静脉显微结扎手术治疗。对照组术后口服左卡尼汀口服液治疗,治疗组术后口服左卡尼汀口服液联合胰激肽原酶肠溶片治疗,所有患者均在术前、术后3个月行精液常规分析,对比两组患者术前术后精液参数中精子前向运动活动率(PR%)、总活动率、精子DNA碎片率以及术后恢复评分。结果 治疗后,治疗组患者精子前向运动活动率、精子总活动率显著优于对照组,差异有统计学意义(P<0.05);治疗后,两组精子DNA碎片对比,差异无统计学意义(P>0.05);治疗组术后恢复情况显著优于对照组,差异有统计学意义(P<0.05);治疗组临床疗效显著优于对照组,差异有统计学意义(P<0.05)。结论 左卡尼汀口服液联合胰激肽原酶肠溶片不仅能改善精索静脉曲张患者术后精子质量,也能加速患者术后症状恢复,可为临床上弱精症伴精索静脉曲张患者提供术后用药新思路。

Objective To observe the efficacy of levocarnitine oral liquid combined with pancreatic kininogenase enteric-coated tablets in the treatment of asthenospermia patients with varicocele after microsurgicalvaricocelectomy.Methods A total of 79 patients with varicocele accompanied by asthenospermia were randomly divided into control group(39 cases) and treatment group(40 cases).All patients were treated with microsurgical varicocelectomy.The control group was treated with levocarnitine oral liquid after operation,and the treatment group was treated with levocarnitine oral liquid combined with pancreatic kininogenase enteric-coated tablets after operation.All patients underwent routine semen analysis before and 3 months after operation.The sperm progressive motility rate(PR%),total motility rate,sperm DNA fragmentation rate and postoperative recovery score were compared between the two groups before and after operation.Results After treatment,the sperm progressive motility rate and total sperm motility rate in the treatment group were significantly better than those in the control group(P<0.05),and there was no significant difference in sperm DNA fragments between the two groups(P>0.05).The postoperative recovery and clinical efficacyof the treatment group were significantly better than those of the control group(P<0.05).Conclusion L-carnitine oral liquid combined with pancreatic kininogenase enteric-coated tablets can not only improve the sperm quality of patients with varicocele after operation,but also accelerate the recovery of postoperative symptoms,which provides new ideas for postoperative medication for patients with asthenospermia and varicocele.
