摘要
目的 探讨lncRNA-GAS5及miR-451在重症肺炎患儿血清中的表达及临床意义。方法 选取2017年1月至2020年1月西北妇女儿童医院收治的89例重症肺炎患儿为研究组,另选取同期健康体检儿童92例作为对照组。通过实时荧光定量聚合酶链反应检测血清lncRNA-GAS5和miR-451水平;通过免疫比浊法检测血清载脂蛋白E(ApoE)及C-反应蛋白(CRP)水平;通过化学发光法检测血清降钙素原(PCT)水平;通过Pearson相关分析lncRNA-GAS5、miR-451表达与ApoE、CRP、PCT水平的相关性;通过受试者工作特征(ROC)曲线分析lncRNA-GAS5及miR-451对重症肺炎的诊断价值。结果 与对照组相比,研究组血清lncRNA-GAS5水平明显降低,miR-451、ApoE、CRP、PCT水平明显升高,差异均有统计学意义(P<0.05);重症肺炎患儿血清中lncRNA-GAS5表达与ApoE、CRP、PCT水平呈负相关(r=-0.493、-0.526、-0.477,P<0.05),miR-451表达与ApoE、CRP、PCT水平呈正相关(r=0.547、0.446、0.454,P<0.05);lncRNA-GAS5与miR-451联合检测诊断重症肺炎的曲线下面积(AUC)高于二者单项检测。结论 lncRNA-GAS5及miR-451可作为早期诊断重症肺炎的分子标志物。
Objective To investigate the expression and clinical significance of lncRNA-GAS5 and miR-451 in serum of children patients with severe pneumonia.Methods Eighty-nine children patients with severe pneumonia in Northwest Women′s and Children′s Hospital from January 2017 to January 2020 were selected as the study group,and 92 healthy children undergoing physical examination in the same period were selected as the control group.The expression levels of serum lncRNA-GAS5 and miR-451 were detected by the real-time fluorescence polymerase chain reaction.The levels of serum apolipoprotein E(ApoE) and C-reactive protein(CRP) were detected by immunoturbidimetry.Serum procalcitonin(PCT) level was detected by chemiluminescence.The Pearson correlation analysis was used to analyze the correlation between lncRNA-GAS5 and miR-451 expression with the ApoE,CRP,PCT levels.The diagnostic value of lncRNA-GAS5 and miR-451 in severe pneumonia was analyzed by the receiver operating characteristic(ROC) curve.Results Compared with the control group,the serum lncRNA-GAS5 level in the study group was significantly decreased,the levels of miR-451,ApoE,CRP and PCT were significantly increased,and the differences were statistically significant(P<0.05).The expression of lncRNA-GAS5 in the children patients with severe pneumonia was negatively correlated with the levels of ApoE,CRP and PCT(r=-0.493,-0.526,-0.477,P<0.05),and the miR-451 expression was positively correlated with the levels of ApoE,CRP and PCT(r=0.547,0.446,0.454,P<0.05).The area under the curve(AUC) of lncRNA-GAS5 combined with miR-451 in the diagnosis of severe pneumonia was higher than that of the two indicators alone.Conclusion lncRNA-GAS5 and miR-451 could be used as the molecular markers for early diagnosis of severe pneumonia.
