MR mDIXON Quant技术定量的冈上肌及冈下肌脂肪浸润与肩袖损伤程度的相关性 被引量:1

Correlation of fatty infiltration of supraspinatus and infraspinatus by MR mDIXON Quant technique and rotator cuff injury degree

摘要 目的观察以MR mDIXON Quant技术定量的冈上肌及冈下肌脂肪浸润与肩袖损伤程度的相关性。方法对82例单侧肩部疼痛伴活动受限患者行肩关节MR常规扫描及mDIXON Quant序列扫描,均显示肩袖损伤(均为冈上肌损伤)。根据MRI冈上肌损伤程度分为退变组、部分撕裂组和全层撕裂组。于mDIXON Quant序列后处理图像上测量冈上肌及冈下肌脂肪分数(FF)。比较3组间FF的差异,并分析冈上肌FF、冈下肌FF及肩袖损伤程度的相关性。结果82例中,退变组31例,部分性撕裂组30例,全层撕裂组21例。全层撕裂组冈上肌FF[(2.09±0.39)%]高于部分撕裂组[(1.81±0.31)%]及退变组[(1.18±0.56)%,P均<0.05],部分撕裂组冈上肌FF高于退变组(P<0.05)。部分撕裂组[(1.61±0.45)%]及全层撕裂组[(1.72±0.61)%]冈下肌FF均高于退变组[(1.04±0.60)%,P均<0.05],部分性撕裂组与全层撕裂组冈下肌FF差异无统计学意义(P>0.05)。冈上肌FF(r s=0.64,P<0.05)及冈下肌FF(r s=0.46,P<0.01)与肩袖损伤程度分别呈强相关及中度相关。冈上肌FF与冈下肌FF呈强相关(r=0.65,P<0.05)。结论以MR mDIXON Quant技术所测冈上肌FF与冈下肌FF有一定相关性,二者与肩袖损伤程度均呈正相关。 Objective To observe the correlation of fatty infiltration of supraspinatus and subaspinatus by MR mDIXON Quant technique and rotator cuff injury degree.Methods Shoulder conventional MR scan and mDIXON Quant sequence scan were performed on 82 patients with unilateral shoulder pain and limited mobility,all showed rotator cuff injury(all were supraspinatus injury).According to the supraspinatus injury degree in MRI,all patients were divided into degeneration group,partial tear group and full-layer tear group.Fat fraction(FF)of supraspinatus and supraspinatus were measured on mDIXON Quant sequence post-processing images.FF among the 3 groups were compared,and the correlation of FF of supraspinatus,supraspinatus and rotator cuff injury degree were analyzed.Results Among the 82 patients,31 cases were in degeneration group,30 cases were in partial tear group and 21 cases were in full-layer tear group.FF of supraspinatus of full-layer tear group([2.09±0.39]%)was higher than that of partial tear group([1.81±0.31]%)and degeneration group([1.18±0.56]%,both P<0.05),and FF of supraspinatus of partial tear group was higher than that of degeneration group(P<0.05).FF of infraspinatus of partial tear group([1.61±0.45]%)and full-layer tear group([1.72±0.61]%)were both higher than that of degeneration group([1.04±0.60]%,both P<0.05),while no significant difference in FF of infraspinatus was found between partial tear group and full-layer tear group(P>0.05).FF of supraspinatus(r s=0.64,P<0.05)and FF of infraspinatus(r s=0.46,P<0.01)were strongly and moderately correlated with rotator cuff injury degree,respectively.FF of supraspinatus was strongly correlated with FF of infraspinatus(r=0.65,P<0.05).Conclusion FF of supraspinatus measured by MR mDIXON Quant technique had certain correlation with FF of supraspinatus,and both were positively correlated with rotator cuff tear degree.
