我国门诊患者就医体验得分率的Meta分析

Meta-analysis of Outpatient Experience Score Rates in China

目的系统评价我国门诊患者就医体验得分率。方法计算机检索中国知网、万方数据、维普资讯、Google Scholar、Web of Science、PubMed数据库,搜集有关门诊患者就医体验的横断面研究,检索时限为2011年10月-2022年5月。由2名研究员独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用Stata16.0软件进行Meta分析。结果共纳入15个横断面研究,包括9267476例门诊患者;Meta分析显示,我国门诊患者就医体验得分率为84.0%(95%CI:82.0%~86.1%);亚组分析结果显示,男性门诊患者就医体验得分率为87.0%,女性门诊患者就医体验得分率为84.3%;29岁及以下门诊患者就医体验得分率为89.8%,30~59岁的门诊患者就医体验得分率为90.4%,60岁及以上的门诊患者就医体验得分率为86.9%;在环境、流程、医护、医患、费用因素方面,门诊患者就医体验得分率分别为83.2%、82.9%、85.9%、86.1%、81.7%。结论我国门诊患者就医体验需要提升。患者性别、年龄、受教育程度、医院所处地区、医院类型、医院环境、医患关系、医疗费用均对患者就医体验有影响。受纳入研究数量和质量的限制,上述结论尚待更多高质量的研究验证。

Objective To systematically evaluate outpatients'medical experience score rates in China.Methods Computer searches of the China Knowledge Network,Wanfang Data,VipInfo,Google Scholar,Web of Science,and PubMed databases were conducted to collect cross-sectional studies on outpatient experience,with a search time frame of October 2011 to May 2022.Meta-analysis was performed using stata16.0 software after two researchers independently screened the literature,extracted information,and evaluated the risk of bias of the included studies.Results A total of 15 cross-sectional studies involving 9267476 outpatients were included.Meta-analysis showed that the score rate of outpatient medical experience in China was 84.0%(95%CI:82.0%-86.1%).The results of subgroup analysis showed that the score rate of medical experience of male outpatients was 87.0%,and that of female outpatients was 84.3%.The score rate of medical experience of outpatients aged 29 and below was 89.8%,the score rate of medical experience of outpatients aged 30-59 was 90.4%,and the score rate of medical experience of outpatients aged 60 and above was 86.9%.In terms of environment,process,medical care,doctor-patient and cost factors,the scores of outpatient medical experience were 83.2%,82.9%,85.9%,86.1%and 81.7%,respectively.Conclusion The current data suggest that the outpatient experience in China needs to be improved.Patient gender,age,education level,region where the hospital is located,hospital type,hospital environment,doctor-patient relationship,and medical costs all have an impact on the outpatient experience in China.Due to the limitation of the number and quality of included studies,the above findings are yet to be verified by more high-quality studies.
