苯巴比妥钠联合硫酸镁在重症破伤风机械通气中的应用

Application of phenobarbital sodium and magnesium sulfate in mechanical treatment ofsevere tetanus ventilation

目的探讨苯巴比妥钠注射液联合硫酸镁注射液在重症破伤风患者机械通气中的应用效果。方法选择2016年4月-2021年10月浙江省淳安县第一人民医院19例、浙江省杭州市第一人民医院32例、浙江省湖州市中心医院25例、浙江省温州市中心医院26例,共计102例重症破伤风患者,采用随机数字表法分为联用硫酸镁组与对照组,各51例。对照组采用苯巴比妥钠注射液,联用硫酸镁组在对照组的基础上采用硫酸镁注射液治疗,观察两组临床疗效,并比较两组苯巴比妥钠注射液日用量、抽搐时间、住院时间、儿茶酚胺代谢情况及并发症发生情况。结果联用硫酸镁组治疗总有效率、去甲肾上腺素、肾上腺素水平均显著高于对照组(P<0.05),苯巴比妥钠注射液日用量、抽搐时间、住院时间均显著低于对照组(P<0.05);两组并发症(肺梗死、肺部感染、肺不张及误吸)发生率比较差异均无统计学意义(P>0.05)。结论在重症破伤风患者机械通气治疗中,采用苯巴比妥钠注射液联合硫酸镁注射液临床疗效较好,并可降低镇定剂用量。

Objective To explore the application effect of phenobarbital sodium injection and magnesium sulfate injection in mechanical ventilation in patients with severe tetanus.Method A total of 102 severe tetanus patients in Zhejiang Province from April 2016 to October 2021 were selected and divided into 51 cases in the observation group and the control group by random number table method.The control group used phenobarbital sodium injection,and the observation group used magnesium sulfate injection on the basis of the control group to observe the clinical efficacy of the two groups,and compared the daily dosage of phenobarbital sodium injection,time of convulsions,hospital stay,catecholamine metabolism and complications between the two groups.Results The total response rate,norepinephrine and adrenaline levels were significantly higher than the control group(P<0.05),and the daily dosage,convulsions time and hospital stay of phenobarbital sodium injection were significantly lower than the control group(P<0.05);the comparison difference of complications(pulmonary infarction,pulmonary infection,atelectasis and aspiration)(P<0.05).Conclusion Phenobarbital sodium injection and magnesium sulfate injection is very effective in mechanical ventilation treatment,and it reduces the amount of tranquilizers.
