KL-6及炎性因子在大鼠外源性脂质性肺炎中的动态表达及临床意义

摘要 目的研究KL-6及炎性因子在大鼠急性外源性脂质性肺炎血清及支气管肺泡灌洗液中的动态表达及临床意义。方法将45只大鼠幼鼠(日龄25-30天)按随机数字表法分为3组,分别为NS组、AELP组、DXM组,每组15只。NS组在直视下于气管插管内滴入0.5mL/kg生理盐水,建模后12、24、48小时腹腔注射生理盐水0.3mL;AELP组、DXM组气管内滴入0.5mL/kg缝纫机油,于建模后12、24、48小时分别腹腔注射生理盐水0.3mL,DXM1mg/kg混合生理盐水配成总量0.3mL。于滴入后1、3、7天每组各取5只,收集血清和支气管肺泡灌洗液标本,分别观察肺组织的病理变化,采用酶联免疫吸附测定法(ELISA)检测血清及肺泡灌洗液中KL-6、IL-6、TNF-α的表达。结果KL-6、IL-6、TNF-α在AELP组、DXM组较NS组在建模后1、3、7天血清表达明显升高,DXM组较AELP组在3、7天表达明显下降,差异有统计学意义(P<0.01)。同样,KL-6、IL-6、TNF-α在AELP组1、3、7天肺泡灌洗液表达较NS组明显升高,DXM组在3、7天肺泡灌洗液中表达较AELP组明显下降,差异有统计学意义(P<0.01)。结论KL-6及炎性因子的表达在大鼠外源性脂质性肺炎血清和肺泡灌洗液中呈动态变化,糖皮质激素的应用可抑制炎症反应,有助于临床判断外源性脂质性肺炎的损伤及治疗情况。 Objective To study the dynamic expression of KL-6 and inflammatory factors in serum and bronchoalveolar lavage fluid(BALF)of rats with acute exogenous liposome pneumonia.Methods 45 young rats(aged 25-30 days)were randomly divided into three groups:NS group、AELP group and DXM group,with 15 rats in each group.The NS group was treated with 0.5mL/kg saline by tracheal intubation under direct vision,and 0.3mL was injected intraperitoneally at 12、24 hand 48h after modeling.0.5mL/kg sewing machine oil was dripped into trachea in AELP group and DXM group.12、24 hand 48h after modeling,normal saline was intraperitoneally injected with 0.3mL or DXM1mg/kg was injected intraperitoneally,mixed with normal saline to form total 0.3mL.5 rats in each group were collected on the 1st,3rd and 7th day after dripping,and the pathological changes of lung tissue were observed.The expression of KL-6、IL-6、TNF-αserum and bronchoalveolar lavage fluid(BALF)was detected by enzyme linked immunosorbent assay(ELISA).Results The serum expressions of KL-6、IL-6 and TNF-αAELP group and DXM intervention group were significantly higher than those in NS group at 1d,three days and seven days after modeling,while those in DXM group were significantly lower than those in AELP group at three days and seven days.Similarly,the expressions of KL-6、IL-6 and TNF-αbronchoalveolar lavage fluid of AELP group were significantly higher than that of NS group at 1d,three days and seven days,while the levels were significantly lower in DXM group than that of AELP group on three days and seven days.Conclusion The expression of KL-6 and inflammatory factors showed dynamic changes in the serum and bronchoalveolar lavage fluid of rats with exogenous liposome pneumonia.The application of glucocorticoid can inhibit the inflammatory reaction and help to judge the injury and treatment of exogenous liposome pneumonia.
