全程责任制助产配合导乐陪伴对产妇分娩结局的干预效果分析

Analysis of intervention effect of whole-responsibility midwifery and doula accompaniment on maternal delivery outcome

目的评价对产妇实施全程责任制助产配合导乐陪伴的应用价值。方法选取120例产妇,根据随机数字表法分为对照组与观察组,每组60例。对照组采用全程责任制助产,观察组在对照组基础上配合导乐陪伴。比较两组干预效果。结果观察组自然分娩率80.0%高于对照组的56.7%,而剖宫产率15.0%低于对照组的33.3%,差异有统计学意义(P<0.05)。两组阴道助产率比较,差异无统计学意义(P>0.05)。观察组第一产程、第二产程、第三产程及总产程均短于对照组,差异有统计学意义(P<0.05)。观察组产后24 h内出血量少于对照组,新生儿窒息发生率低于对照组,差异有统计学意义(P<0.05)。结论将全程责任制助产配合导乐陪伴分娩模式应用于产妇分娩中效果显著,提高了分娩质量,值得临床推广与应用。

Objective To evaluate the application value of midwifery and doula accompaniment for puerpera with wholecourse responsibility system.Methods 120 parturients were selected and divided into control group and observation group according to random number table method,with 60 cases in each group.The control group received whole-process responsibility midwifery,and the observation group was accompanied by doula on the basis of the control group.The intervention effect of the two groups was compared.Results The natural delivery rate of the observation group was 80.0%higher than that of the control group(56.7%),while the cesarean section rate of 15.0%was lower than that of the control group(33.3%),and the difTerence was statistically significant(P<0.05).There was no significant difference in the rate of vaginal midwifery between the two groups(P>0.05).The first stage of labor,the second stage of labor,the third stage of labor and the total stage of labor in the observation group were shorter than those in the control group,with statistical significance(P<0.05).The amount of postpartum bleeding in the observation group was less than that in the control group within 24 h,and the incidence of neonatal asphyxia was lower than that in the control group,with statistical significance(P<0.05).Conclusion The whole responsibility system of midwifery and doula accompanied delivery mode applied in maternal delivery has a remarkable effect,improves the quality of delivery,and is worthy of clinical promotion and application.
