摘要
目的:观察七氟醚联合舒芬太尼在成人麻醉诱导中的应用效果。方法:选取2018年8月至2019年8月该院收治的100例行气管插管全身麻醉手术患者进行前瞻性研究,采用随机数字表法将其分为观察组与对照组各50例。对照组给予舒芬太尼麻醉诱导,观察组给予七氟醚联合舒芬太尼麻醉诱导,比较两组麻醉效果(包括意识消失时间、符合气管插管条件时间、术后拔管时间)、不同时间[麻醉诱导前(T_(0))、插管时(T_(1))、插管后1 min(T_(2))、插管后5 min(T_(3))]血流动力学指标[平均动脉压(MAP)、心率]水平和术后躁动情况[Riker镇静-躁动量表(SAS)]评分。结果:观察组意识消失时间、符合气管插管条件时间、术后拔管时间均短于对照组,差异有统计学意义(P<0.05);T_(0)时,两组MAP、心率水平比较,差异均无统计学意义(P>0.05);T_(1)~T_(2)时,两组MAP水平均高于T_(0),但观察组低于对照组,差异有统计学意义(P<0.05);T_(3)时,对照组MAP水平高于T_(0)时,且高于观察组,差异有统计学意义(P<0.05);T_(1)~T_(3)时,观察组心率水平与T_(0)时比较,差异均无统计学意义(P>0.05);T_(1)~T_(3)时,对照组心率水平均高于T_(0)时,且高于观察组,差异有统计学意义(P<0.05);手术结束时及术后5、10 min,观察组SAS评分均低于对照组,差异有统计学意义(P<0.05)。结论:七氟醚联合舒芬太尼应用于成人麻醉诱导可缩短意识消失时间、符合气管插管条件时间和术后拔管时间,改善血流动力学指标水平,降低术后SAS评分,效果优于单纯舒芬太尼麻醉诱导。
Objective:To observe application effects of Sevoflurane combined with Sufentanil in adult anesthesia induction.Methods:A prospective study was conducted on 100 patients undergoing general anesthesia with tracheal intubation admitted to the hospital from August 2018 to August 2019.They were divided into observation group and control group by using the random number table method,50 cases in each.The control group was given Sufentanil anesthesia induction,while the observation group was given Sevoflurane combined with Sufentanil anesthesia induction.The anesthetic effects (including the time of loss of consciousness,the time of meeting the conditions of tracheal intubation,the time of extubation after the surgery),the levels of hemodynamic indexes[mean arterial pressure (MAP),heart rate (HR)]at different time[before anesthesia induction (T_(0)),at intubation (T_(1)),1 min after intubation (T_(2)),5 min after intubation (T_(3))],and the postoperative agitation score[Riker sedation-agitation scale (SAS)]were compared between the two groups.Results:The time of consciousness disappearance,the time of meeting the conditions of tracheal intubation and the time of extubation in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).At T_(0),there were no significant differences in the MAP and HR levels between the two groups (P>0.05).There were no significant differences in the HR level between T_(1)-T_(3)and T_(0)in the observation group (P>0.05).At T_(1)-T_(3),the HR levels of the control group were higher than those at T_(0),and higher than those of the observation group,and the differences were statistically significant (P<0.05).At T_(1)-T_(2),the MAP levels of the two groups were higher than those at To,but those in the observation group were lower than those in the control group,and the differences were statistically significant (P<0.05).At T_(3),the MAP level of the control group was higher than that at T_(0),and higher than that of the observation group,and the differences were statistically significant (P<0.05).Further,at the end of the surgery and 5 and 10 min after the surgery,the SAS scores of the observation group were lower than those of the control group,and the differences were statistically significant (P<0.05).Conclusions:Sevoflurane combined with Sufentanil in adult anesthesia induction can shorten the time of loss of consciousness,the time of meeting the conditions of tracheal intubation and the time of extubation after the surgery,improve the levels of hemodynamic indexes and reduce the postoperative SAS score.Moreover,it is superior to single Sufentanil anesthesia induction.
