肩峰下注射玻璃酸钠联合肩关节镜下肩袖修补术治疗肩袖损伤患者的效果

Effects of subacromial injection of sodium hyaluronate combined with arthroscopic rotator cuff repair on patients with rotator cuff injuries

目的:观察肩峰下注射玻璃酸钠联合肩关节镜下肩袖修补术治疗肩袖损伤患者的效果。方法:选取2019年1月至2020年1月该院收治的60例肩袖损伤患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各30例。对照组行肩关节镜下肩袖修补术治疗,观察组在对照组基础上联合肩峰下注射玻璃酸钠治疗。术后随访6个月,比较两组术前和术后24 h疼痛程度[视觉模拟评分法(VAS)]评分、术前和术后6个月肩关节功能(Constant-Murley肩关节功能评分法)评分、患侧肩关节活动度,以及并发症发生率。结果:术后,观察组VAS评分低于对照组,差异有统计学意义(P<0.05);观察组Constant-Murley肩关节功能评分和患侧肩关节外展、前屈、后伸、内旋、外旋角度均高于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:肩峰下注射玻璃酸钠联合肩关节镜下肩袖修补术治疗肩袖损伤患者,可减轻术后疼痛,促进肩关节功能恢复,改善肩关节活动度,效果优于单纯肩关节镜下肩袖修补术治疗。

Objective:To observe effects of subacromial injection of sodium hyaluronate combined with arthroscopic rotator cuff repair on patients with rotator cuff injuries.Methods:A prospective study was conducted on 60 patients with rotator cuff injuries admitted to the hospital from January 2019 to January 2020.They were divided into control group and observation group according to the random number table method,30 cases in each.The control group was treated with arthroscopic rotator cuff repair,while the observation group was treated with subacromial injection of sodium hyaluronate on the basis of that of the control group.These patients were followed up for 6 months.The pain scores[visual analogue scale(VAS)]before and 24 h after the surgery,the shoulder joint function(Constant-Murley shoulder function score)score before and 6 months after the surgery,the range of motion of the affected shoulder joint,and the complications were compared between the two groups.Results:After the surgery,the VAS score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).The Constant-Murley shoulder function scores and the abduction,anteflexion,posterior extension,internal rotation and external rotation angles of the affected shoulder joint in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:Subacromial injection of sodium hyaluronate combined with arthroscopic rotator cuff repair for the patients with rotator cuff injuries can reduce the postoperative pain,promote the recovery of the shoulder joint function and improve the range of motion of the shoulder joint.Moreover,it is superior to single arthroscopic rotator cuff repair.
