出院准备服务模式对老年AF导管射频消融术后患者自我管理水平的影响研究

Influence of discharge preparation service mode on self-management level of elderly patients with atrial fibrillation after catheter radiofrequency ablation

目的探究出院准备服务模式在老年心房颤动(AF)导管射频消融术后患者自我管理水平的影响。方法选择行导管射频消融术治疗的200例老年AF患者为研究对象,按数表法将其分为对照组与观察组,每组100例。给予对照组心内科常规护理和出院后随访,观察组则在对照组的基础上添加出院准备服务模式,对2组患者的出院准备度、出院指导质量、自我管理水平及出院3个月内的再住院率进行比较。结果观察组出院准备度量表(RHDS)中的疾病知识、自身状况、可获得的社会支持、出院后的应对能力评分均高于对照组(P<0.05);观察组出院指导质量量表(QDTS)中需要的内容、获得的内容、指导技巧及效果评分均高于对照组(P<0.05);观察组的自我管理行为调查表中遵医用药、定期检测和健康生活方式的评分均高于对照组(P<0.05);观察组出院3个月内的再住院率低于对照组(P<0.05)。结论出院准备服务模式对老年AF导管射频消融术后患者的出院准备度、出院指导质量和自我管理水平均具有积极影响,能减少其非计划性再住院率。

Objective To explore the effect of discharge preparation service mode on self-management level of elderly patients with atrial fibrillation(AF)after catheter radiofrequency ablation.Methods A total of 200 elderly patients with AF who underwent catheter radiofrequency ablation in our hospital were selected as the research subjects.They were randomly divided into the control group and the observation group,with 100 cases in each group.Conventional cardiological nursing and discharge follow-up were given to patients in both groups,and those in the observation group were additionally intervened by the discharge preparation service mode.The level of discharge preparation,discharge guidance quality,self-management level,and readmission rate within 3 months of discharge were compared between the two groups.Results The scores of disease knowledge,self-condition,available social support,and coping ability after discharge of readiness for hospital discharge scale(RHDS)in the observation group were significantly higher than those in the control group(all P<0.05).The scores of content needed,obtained content,and guidance skills in the quality of discharge guidance scale(QDTS),and the efficacy in the observation group were significantly higher than those of the control group(all P<0.05).The scores of compliance with medication,regular testing and healthy lifestyle of the self-management behavior questionnaire in the observation group were significantly higher than those in the control group(all P<0.05).The readmission rate within 3 months of discharge in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The discharge preparation service mode has a positive impact on the discharge preparation,discharge guidance quality and self-management level of elderly patients with AF after catheter radiofrequency ablation,which significantly reduces the unplanned readmission rate.
