sdLDL-C、NLR结合ACR评估糖尿病早期肾损伤的临床价值

The Clinical Value of sdLDL-C,NLR Combined with ACR in Assessing Early Diabetic Kidney Injury

目的探讨血清小而密低密度脂蛋白胆固醇(sdLDL-C)、中性粒细胞与淋巴细胞比值(NLR)结合尿微量白蛋白/尿肌酐比值(ACR)评估糖尿病早期肾损伤中的临床价值。方法选取本院2020年1月至2022年6月期间收治的100例2型糖尿病早期肾损伤患者,将其设为观察组,并选取同期诊治的单纯2型糖尿病患者100例,将其设为对照组,均采集两组患者静脉血及留取尿液样本后,测定sdLDL-C、NLR、ACR水平,分析3种指标对糖尿病早期肾损伤的诊断价值。结果观察组患者中性粒细胞计数、NLR水平高于对照组,尿微量白蛋白(mALB)、ACR、SDLDL-C水平均高于对照组,差异有统计学意义(P<0.05)。ROC曲线绘制后,sdLDL-C、NLR结合ACR诊断糖尿病早期肾损伤的灵敏度为94.00%、特异性为91.00%,均高于单独指标的诊断价值,差异有统计学意义(P<0.05)。结论sdLDL-C、NLR结合ACR诊断2型糖尿病早期肾损伤的临床价值高,可为疾病早期诊断、病情监测提供合理的参考依据,值得临床应用。

Objective To investigate the clinical value of serum small and dense low-density lipoprotein cholesterol(SDLDL-C),neutrophil-to-lymphocyte ratio(NLR)combined with urinary albumin/urine creatinine ratio(ACR)in the assessment of early diabetic kidney injury.Methods 100 patients with early diabetic kidney injury admitted to our hospital from January,2020 to June,2022 were selected and set as the observation group,while 100 patients with simple diabetes who were diagnosed and treated during the same period were selected and set as the control group.After collecting venous blood and urine samples from the two groups of patients,levels of sdLDL-C,NLR,and ACR were determined,and the diagnostic values of these three indicators in early diabetic kidney injury were analyzed.Results The neutrophil count and NLR level in the observation group were higher than those in the control group,and the urinary microalbumin(mALB),ACR,and SDLDL-C levels were higher than those in the control group(P<0.05).ROC curve was drawn and it showed that the sensitivity and specificity of sdLDL-C and NLR combined with ACR in the diagnosis of early diabetic kidney injury were 94.00%and 91.00%,respectively,which were higher than the diagnostic value of the individual indicators(P<0.05).Conclusion The combination of sdLDL-C and NLR with ACR has a high clinical value in the diagnosis of early diabetic kidney injury,which can provide a reasonable reference for the early diagnosis and disease monitoring,and is worthy of clinical application.
