摘要
目的探讨锌指蛋白56(ZNF56)在直肠神经内分泌肿瘤(RNENs)中的表达及其临床意义。方法选择112例RNENs患者为研究对象,收集肿瘤组织及正常组织,采用RT-PCR、免疫组化染色、Western blot检测肿瘤组织中ZNF56 mRNA及蛋白的表达情况,收集患者临床资料并进行随访,统计分析ZNF56表达的临床意义。结果(1)112例RNENs患者中,ZNF56的总体阳性率为69.6%(78/112),G1级、G2级、G3级患者的阳性率分别为65.6%(42/64)、71.4%(20/28)、80.0%(16/20)。Syn、CgA、NSE的总体阳性率为100%(112/112)、75.8%(85/112)、64.3%(72/112)。(2)Spearman等级相关分析显示,ZNF56 mRNA的表达水平与Syn mRNA(r=0.515,P<0.001)、CgA mRNA(r=0.489,P<0.001)、NSE mRNA(r=0.432,P=0.006)均呈显著的正相关关系。(3)RT-PCR、Western blot检测显示,正常组织、直肠神经内分泌肿瘤组织中ZNF56 mRNA的相对表达量为1.32±0.73、5.79±0.86,ZNF56蛋白的相对表达量为1.89±0.66、7.20±0.75,差异均具有统计学意义(P<0.05)。(4)ZNF56阳性表达患者的5年生存率为15.3%(12/78),阴性表达患者为34.2%(12/35)。Cox多因素分析结果显示,ZNF56表达(χ^(2)=5.136,P<0.001)、肿瘤分级(χ^(2)=14.685,P<0.001)及肌层浸润(χ^(2)=2.365,P=0.031)是影响直肠神经内分泌肿瘤的独立危险因素。结论ZNF56在直肠神经内分泌肿瘤组织中显著高表达,且对患者预后有明显的不良影响。
Objective To investigate the expression and clinical significance of ZNF56 in rectal neuroendocrine tumors.Methods 112 patients with rectal neuroendocrine tumors were selected as the research objects.Both tumor tissues and normal tissues were collected to detect the expression of ZNF56 mRNA and protein,and the clinical significance of ZNF56 expression was statistically analyzed.Results(1)Among 112 patients,the overall positive rate of ZNF56 was 69.6%.The overall positive rates of syn,CGA and NSE were 100%,75.8%and 64.3%,respectively.(2)Spearman rank correlation analysis showed that the expression level of ZNF56 mRNA was significantly positively correlated with levels of syn mRNA,CGA mRNA and NSE mRNA.(3)Compared with normal tissues,the relative expressions of ZNF56 mRNA and ZNF56 protein in rectal neuroendocrine tumors were 1.32±0.73,5.79±0.86 and 1.89±0.66,7.20±0.75,respectively.(4)5-year survival rate of patients with positive ZNF56 expression was 15.3%(12/78),and that of patients with negative ZNF56 expression was 34.2%(12/35).The difference was statistically significant.Cox multivariate analysis showed that ZNF56 expression,tumor grade and myometrial infiltration were independent risk factors for rectal neuroendocrine tumors.Conclusion ZNF56 is highly expressed in rectal neuroendocrine tumors,and has a significant adverse effect on the prognosis of patients.
