摘要
目的 报道一例白塞氏病合并急性单核细胞性白血病的病例并文献回顾。方法 通过全身体检、抗核抗体检查、骨髓形态学检查和白血病免疫分型进行等诊断。查阅相关文献,回顾此病白塞氏病是以血管炎为基础的慢性系统性自身免疫性疾病,这是一种以口腔反复溃疡、生殖器溃疡和葡萄膜炎为主要表现的三联综合征,主要的临床表现除常见复发性口腔溃疡、眼部病变、皮肤损害和生殖器溃疡外,关节、大、中血管也可受累,出现多系统、多脏器的损坏。急性单核细胞性白血病是急性髓系白血病的一种,其临床表现为贫血、发热、皮肤、黏膜的改变,如齿龈增生、肿胀、出血、溃疡及坏死,其皮肤损坏可表现为丘疹、结节、肿块等。结果 该病例符合白塞氏病合并急性单核细胞性白血病发病特点。结论 本文报道了一例白塞氏病合并急性单核细胞性白血病的病例,其临床表现和实验室检查的结果,为临床及时作出诊断提供依据,避免造成误诊漏诊,有助于患者早日治疗。
Objective This paper aims to report a case of Behcet’s disease with acute monocytic leukemia and retrospectively conduct literature review. Methods The diagnosis was carried out with physical examination, antinuclear antibody test, bone marrow morphology test and leukemia immunotyping. Refer to relevant literature and review that Behcet’s disease is a chronic systemic autoimmune disease based on vasculitis. It is characterized by recurrent oral ulcers, genital ulcers and uveitis. In addition to the common recurrent oral cavity ulcers, eye lesions, skin lesions and genital ulcers, the main clinical manifestations of Behcet’s disease can also be affected by joints, large and medium blood vessels, resulting in multi-system and multi-organ damage. Acute mononuclear leukemia is a kind of acute myeloid leukemia. Its clinical manifestations include anemia, fever, skin and mucosal changes, such as gingival hyperplasia, swelling, bleeding, ulcer and necrosis, and its skin damage can be manifested as papules, nodules, lumps, etc. Results This case was consistent with the characteristics of Behcet’s disease with acute monocytic leukemia. Conclusion This paper reports a case of Behcet’s disease with acute mononuclear leukemia. Its clinical manifestation and laboratory examination results provide basis for clinical diagnosis in time, avoid misdiagnosis and missed diagnosis, and contribute to early treatment.
