经鼻高流量氧疗辅助治疗重症肺炎合并呼吸衰竭的效果评价

Effect assessment of transnasal high flow oxygen therapy on severe pneumonia complicated with respiratory failure

目的:探讨经鼻高流量氧疗辅助治疗重症肺炎合并呼吸衰竭的效果。方法:纳入2019年6月—2022年1月江西省抚州市东乡区人民医院收治的重症肺炎合并呼吸衰竭患者50例。按照随机数字表法分为两组,每组25例。两组均接受常规治疗,对照组辅助高流量面罩吸氧治疗,观察组辅助经鼻高流量氧疗。两组均连续治疗1周。比较两组患者的临床疗效、主要症状消失时间以及治疗前后呼吸频率(RR)、氧合指数(OI)、血氧饱和度(SaO_(2))、动脉血氧分压(PaO_(2))、C反应蛋白(CRP)和降钙素原(PCT)检测结果的差异。结果:观察组治疗总有效率为96.00%,高于对照组的76.00%(P<0.05);观察组气促、肺部啰音、发绀、三凹征消失时间均短于对照组(P<0.05);RR检测结果低于对照组(P<0.05),OI、SaO_(2)、PaO_(2)检测结果均高于对照组(P<0.05);观察组治疗后CRP、PCT检测结果均低于对照组(P<0.05)。结论:经鼻高流量氧疗辅助治疗重症肺炎合并呼吸衰竭能够提升治疗预后,促进症状缓解,值得临床推荐。

Objective:To investigate the effect of transnasal high flow oxygen therapy on severe pneumonia complicated with respiratory failure.Methods:Fifty patients with severe pneumonia complicated with respiratory failure treated in Dongxiang District People’s Hospital,Fuzhou City,Jiangxi Province from June 2019 to January 2022 were included.According to the random number table method,they were divided into two groups with 25 cases in each group.Both groups received routine treatment,the control group was assisted with high flow mask oxygen inhalation,and the observation group was assisted with transnasal high flow oxygen therapy.Both groups were treated continuously for 1 week.The clinical efficacy,disappearance time of main symptoms,respiratory rate(RR),oxygenation index(OI),blood oxygen saturation(SaO_(2)),arterial partial pressure of oxygen(PaO_(2)),C-reactive protein(CRP)and procalcitonin(PCT)before and after treatment were compared between the two groups.Results:The total effective rate in the observation group was 96.00%,which was higher than that in the control group 76.00%;the disappearance time of shortness of breath,lung rale,cyanosis and three concave signs in the observation group was shorter than that those in the control group;the test results of RR in the observation group were lower than that in the control group(P<0.05),and the test results of OI,SaO_(2) and PaO_(2) in the observation group were higher than those in the control group(P<0.05);the results of CRP and PCT in the observation group were lower than those in the control group after treatment(P<0.05).Conclusion:The adjuvant treatment of severe pneumonia complicated with respiratory failure with transnasal high flow oxygen therapy can improve the prognosis and promote the relief of symptoms,which is worthy of clinical recommendation.
