青少年颞下颌关节紊乱病应用MR T_(2)-Mapping成像评估关节盘损伤的研究

Evaluation of Articular Disc Injury Using MR T_(2)-Mapping Imaging in Adolescent Patients with Temporomandibular Disorder

目的探讨青少年颞下颌关节紊乱病患者应用磁共振(magnetic resonance,MR)T_(2)-Mapping成像评估颞下颌关节盘形态学的变化并进行定量分析。方法收集笔者医院2019年4月~2020年10月确诊的青少年颞下颌关节紊乱病患者,其中可复性颞下颌关节紊乱患者45例(可复性组)、不可复性颞下颌关节紊乱患者45例(不可复性组)作为病例组,并选取同期健康青少年45例作为对照组。所有受检者颞下颌关节均进行颞下颌关节盘磁共振成像(magnetic resonance imaging,MRI)、MR T_(2)-Mapping成像,比较3组颞下颌关节盘MRI、MR T_(2)-Mapping伪彩图形态学结构变化及定量指标。结果3组间MR T_(2)-Mapping定量指标比较,差异有统计学意义(P<0.01),病例组指标明显低于对照组,不可复性组明显低于可复性组,且在MR T_(2)-Mapping伪彩图的形态结构表现不同。结论青少年颞下颌关节紊乱病可通过应用MR T_(2)-Mapping序列对关节盘进行T_(2)值的测量,更精确评估颞下颌关节紊乱的程度,为临床工作中对该疾病的分类、诊断、干预后评估提供可靠的影像学依据。

Objective To investigate the morphological changes of the temporomandibular joint discs in adolescents with temporomandibular disorders(TMD)assessed by MR T_(2)-Mapping sequences and to perform quantitative analysis.Methods A total of 90 adolescents with TMD diagnosed from April 2019 to October 2020 were collected as the case group,including 45 patients with reversible TMD and 45 patients with irreversible TMD.and 45healthy adolescents were selected as the control group in the same period.Magnetic resonance imaging(MRI)and MR T_(2)-Mapping sequence were performed on the temporomandibular joint disc of all subjects,The morphological changes and quantitative parameters of temporomandibular jiont disc on MR and MR T_(2)-Mapping pseudo-color images were compared among the three groups.Results There were statistically significant differences of MR T_(2)-Mapping quantitative indexes among the three groups(P<0.01).The indexes of the TMD group were significantly lower than those in the normal temporomandibular joint group,and the irreversible TMD group was significantly lower than that of reversible TMD group,the morphology and structure of pseudo-color images in MR T_(2)-Mapping were different.Conclusion MR T_(2)-mapping sequence can be used to measure the T_(2)value of temporomandibular joint disc in adolescents with TMD,which can more accurately evaluate the degree of TMD,and provide reliable imaging basis for the clinical classification,diagnosis and post-intervention evaluation of the disease in clinical work.
