脐部多发性大汗腺汗囊瘤1例

Multiple Apocrine Hidrocystoma in the Navel: A Case Report

患者女,28岁,脐部黑色肿物8个月。皮肤科情况:脐部数个黑色半球形隆起的囊性结节,半透明状,质地中等,无压痛。皮损组织病理示:表皮大致正常,真皮内可见多个大囊腔,囊腔周围有纤维性假膜,囊壁由1~2层细胞构成,内层细胞呈柱状,可见顶浆分泌现象,外层为扁平或立方形细胞。免疫组织化学染色示:CK(+),CK7(+),CK20(-),囊腔内侧上皮细胞EMA(+),囊腔外侧上皮细胞S100(+)。诊断:多发性大汗腺汗囊瘤。手术完整切除后随访中。

A 28-year-old female presented with black masses in the navel for 8 months.Dermatological examination showed that several black hemispherically raised and translucent cystic nodules without tenderness were seen in the navel which were medium in texture.Histopathology of the lesion showed that the epidermis was roughly normal and there were several large cystic cavities in the dermis surrounded by fibrous pseudomembrane.The lumen was lined with two or more layers of epithelial cells.The outer cells were flat which showed apocrine secretion, and the inner nuclei were round or roundish.Immunohistochemical staining showed that CK and CK7 were positive, and epithelial cells on the inner side of the lumen were positive for EMA.Lateral lumen epithelial cells were positive for S100.The diagnosis of multiple apocrine hidrocystoma was made.Complete resection was performed and follow-up was conducted at present.
