基于磁共振分析3D-ASL对颅脑肿瘤血流灌注评估价值

Value of Three-Dimensional Arterial Spin-Labeled Perfusion Magnetic Resonance Imaging in Evaluating Brain Tumor Perfusion

摘要 目的 分析基于磁共振分析3D-ASL对颅脑肿瘤血流灌注评估价值。方法 择医院2019年1月至2021年1月诊治颅脑肿瘤患者70例。所有患者先接受MRI(GE公司型号MR750 3.0T核磁共振扫描仪)常规检查后注射对比剂进行对比增强MRI(DSC-MRI)扫描,再进行3D-ASL。比较高级别和低级别患者肿瘤和对侧半球CBF数值。以病理检查结果为金标准,计算DSC-MRI、3D-ASL检出和准确情况。结果 高级别肿瘤和对侧半球CBF数值分别高于低级别(P<0.05)。DSC-MRI、3D-ASL的脑肿瘤检出率差异比较无统计学意义(P>0.05),其中3D-ASL准确率71.43%(低级别)、88.87%(高级别)分别高于DSC-MRI(P<0.05)。结论 高级别颅脑肿瘤肿瘤和对侧半球CBF数值较高,可能提升颅脑肿瘤血流灌注评估价值,推荐使用。 Objective To investigate the value of three-dimensional arterial spin-labeled(3D-ASL)perfusion magnetic resonance imaging(MRI)in evaluating brain tumor perfusion.Methods Seventy patients with brain tumors in our hospital from January 2019 to January 2021 were enrolled, and all patients received MRI(3.0 Tesla Magnetic Resonance Scanner, GE MR750,GE Healthcare),dynamic susceptibility contrast-enhanced MRI(DSC-MRI),and 3D-ASL examinations successively.The cerebral blood flow(CBF)of tumor ipsilateral cerebellar hemisphere and contralateral cerebellar hemisphere was recorded.Taking the pathological examination results as gold standard, the detection and accuracy rate of DSC-MRI and 3D-ASL were calculated.Results The CBF values of high-grade tumors and contralateral hemisphere were higher than those of low-grade tumors(P<0.05).The detection rate of brain tumors showed no significant difference between DSC-MRI and 3D-ASL(P>0.05).The accuracy of 3D-ASL in low-grade and high-grade tumors was 71.43% and 88.87%,which was higher than that of DSC-MRI(P<0.05).Conclusion CBF values of high-grade brain tumors and contralateral hemisphere are high, which may improve the evaluation value of blood perfusion of brain tumors.
