miR-345-5p、chemerin、HDL-C在HLAP患者中的表达及与HLAP发病的相关性研究

Expression and correlation of miR-345-5p,chemerin and HDL-C in patients with HLAP and their correlation with HLAP morbidity

目的探讨高脂血症性急性胰腺炎(HLAP)患者血清趋化素(chemerin)、miR-345-5p、高密度脂蛋白胆固醇(HDL-C)的表达及与HLAP发病的相关性。方法回顾性分析2020年6月至2021年12月于该院确诊的HLAP患者100例作为HLAP组,另选取同时期来该院行一般体检的健康人50例作为对照组。采用酶联免疫吸附试验检测血清chemerin水平,实时荧光定量PCR法检测血清miR-345-5p水平,全自动生化分析仪检测血清HDL-C。对比两组患者血清miR-345-5p、chemerin、HDL-C水平差异;探讨两组患者血清其他指标如肿瘤坏死因子(TNF-α)、降钙素原(PCT)及C反应蛋白(CRP)的表达水平差异;采用Spearman相关系数分析miR-345-5p、chemerin、HDL-C与HLAP发病的相关性;绘制受试者工作特征(ROC)曲线评估miR-345-5p、chemerin、HDL-C联合检测对HLAP的诊断效能。结果HLAP组患者血清miR-345-5p、chemerin水平显著高于对照组,而HDL-C显著低于对照组,差异有统计学意义(P<0.05);HLAP组患者血清TNF-α、PCT、CRP水平显著高于对照组,差异有统计学意义(P<0.05);Spearman相关系数分析显示,血清miR-345-5p、chemerin水平与HLAP的发病均呈正相关(r=0.518、0.604,P<0.001),而HDL-C与HLAP发病呈负相关(r=-0.672,P<0.001)。ROC曲线显示,联合检测的曲线下面积为0.886,特异度为87.50%,灵敏度为86.50%,高于任意单一检测结果,具有良好的诊断效能(P<0.05)。结论血清miR-345-5p、chemerin、HDL-C联合检测对诊断HLAP具有重要意义,值得临床推广应用。

Objective To investigate the expression of chemerin,miR-345-5p and high density lipoprotein cholesterol(HDL-C)in patients with hyperlipidemic acute pancreatitis(HLAP)and their correlation with HLAP morbidity.Methods A retrospective analysis was performed on 100 patients diagnosed with HLAP admitted to the hospital from June 2020 to December 2021 as the HLAP group,and 50 healthy people who came to the hospital for general physical examination during the same period as the control group.Serum chemerin level was detected by ELISA.Serum miR-345-5p level was detected by real-time fluorescence quantitative PCR,and serum HDL-C was detected by automatic biochemical analyzer.Serum miR-345-5p,chemerin and HDL-C levels were compared between the two groups.The expression levels of other serum indicators such as tumor necrosis factor(TNF-α),procalcitonin(PCT)and C-reactive protein(CRP)in 2 groups were investigated.Spearman correlation coefficient was used to analyze the correlation between miR-345-5p,chemerin,HDL-C and HLAP incidence.Receiver operating characteristic(ROC)curve was drawn to evaluate the diagnostic efficacy of combined miR-345-5p,chemerin,and HDL-C detection for HLAP.Results Serum miR-345-5p and chemerin levels in HLAP group were significantly higher than those in control group,while HDL-C level was significantly lower than that in control group,the differences were statistically significant(P<0.05).The levels of serum TNF-α,PCT and CRP in HLAP group were significantly higher than those in control group,and the differences were statistically significant(P<0.05).Spearman correlation coefficient analysis showed that serum miR-345-5p and chemerin levels were positively correlated with the incidence of HLAP(r=0.518,0.604,P<0.001),while HDL-C was negatively correlated with the incidence of HLAP(r=-0.672,P<0.001).ROC curve showed that the area under the combined detection curve was 0.886,the specificity was 87.50%,and the sensitivity was 86.50%,which was higher than any single detection result and had good diagnostic efficacy(P<0.05).Conclusion The combined detection of serum miR-345-5p,chemerin and HDL-C has important significance in the diagnosis of HLAP,and is worthy of clinical application.
