摘要
目的探讨短程团体失眠认知行为疗法(CBT-I)在慢性失眠患者护理中的应用效果。方法将佛山市第三人民医院2021年3月—6月接收的80例慢性失眠患者采用随机数字表法分为对照组与研究组,每组各40例。对照组采用药物治疗,研究组采用短程CBT-I治疗,以6~8名患者与干预者组成干预团体,开展3次共6周的短程CBT-I。采用多导睡眠图(PSG)监测患者入组时及干预2个月后的总睡眠时间(TST)、睡眠效率(SE)、快速眼动睡眠(REM)时间占比与非快速眼动睡眠(NREM)时间占比;采用Flinders疲劳量表(FFS)、简化版睡眠相关不良信念与态度量表(DBAS-16)评价患者入组时及干预2个月后的疲劳程度、睡眠信念与态度。结果研究组入组2个月时的TST、SE显著高于同期对照组,差异有统计学意义(P<0.05)。研究组入组2个月时的FFS评分低于同期对照组,差异有统计学意义(P<0.05)。研究组入组2个月时的DBAS-16量表各维度评分及总分均高于同期对照组,差异有统计学意义(P<0.05)。结论应用短程团体失眠认知行为疗法可改善慢性失眠患者的睡眠状况,减轻其疲劳程度,增强其睡眠信念与态度。
Objective To explore the application efficacy of short-term group cognitive behavioral therapy for insomnia(CBT-I)in patients with chronic insomnia.Methods 80 patients with chronic insomnia admitted from the The Third People′s Hospital of Foshan City from March to June 2021 were randomly divided into the control group and the study group,each group contained with 40 cases;the control group were treated with drugs;the study group were treated with short-term CBT-I,with 6-8 patients and interveners formed an intervention group to carry out 3 times short-term CBT-I for 6 weeks.The Polysomnography(PSG)was used to monitor the total sleep time(TST),sleep efficiency(SE),rapid eye movement sleep(REM)time and non-rapid eye movement sleep(NREM)time at the time of enrollment and 2 months after intervention.The Flinders Fatigue Scale(FFS),simplified version of Dysfunctional Beliefs and Attitudes about Sleep Scale 16 version(DBAS-16)were used to evaluated the degree of fatigue,sleep beliefs and attitudes at the time of enrollment and 2 months after intervention.Results The TST and SE of the study group for 2 months after enrollment were significantly higher than that of the control group during the same period,and the difference were statistically significant(P<0.05).The FFS score of the study group for 2 months were lower than that of the control group,and the difference were statistically significant(P<0.05);The scores and total scores of each dimension of the DBAS-16 scale of the study group for 2 months were higher than the control In the same period,the difference were statistically significant(P<0.05).Conclusions The Short-term group CBT-I can improve the sleep status of patients with chronic insomnia,reduce fatigue,and enhance sleep belief and attitude.
