慢性阻塞性肺疾病急性加重病人出院随访的最佳证据总结

Summary of best evidence for discharge follow-up of patients with acute exacerbations of chronic obstructive pulmonary disease

目的:总结慢性阻塞性肺疾病急性加重(AECOPD)病人出院随访的最佳证据,为临床护理人员提供参考。方法:依据“6S”证据模型,系统检索Up To Date、BMJ Best Practice、澳大利亚Joanna Briggs Institute循证卫生保健中心、the Cochrane Library、英国胸科协会(BTS)、加拿大胸科协会(CTS)、PubMed、Web of Science、EMbase、中国生物医学文献数据库、中国知网和万方数据库等关于AECOPD病人出院随访的证据,包括临床决策、指南、系统评价、专家共识、证据总结等。检索时限为建库至2021年11月15日。由2名研究人员对文献质量进行评价,并对符合纳入、排除标准的文献进行证据提取和汇总。结果:共纳入12篇文献,包括临床决策1篇、指南5篇、专家共识2篇、系统综述2篇、证据总结2篇,总结出随访人员、随访方式、随访频率、随访评估内容、评估和提高自我管理能力、健康教育6个主题共22条证据。结论:本研究总结的AECOPD病人出院随访的最佳证据,医护人员在使用证据时需结合病人实际情况进行综合判断,采取最佳的护理措施,以科学的方法对病人进行出院后随访管理。

Objective:To summarize the best evidence of discharge follow-up of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD),and to provide reference for clinical nurses.Method:According to the"6S"evidence model,clinical decisions,guidelines,systematic reviews,expert consensus,and summaries of evidence regarding discharge follow-up of AECOPD patients were reviewed from Up To Date,BMJ Best Practice,Australia JBI evidence-based health care center,and the Cochrane Library,BTS,CTS,PubMed,Web of Science,EMbase,CBM,CNKI,WanFang Database and other databases.The retrieval period was from the establishment of the database to November 15,2021.The quality of the literature was evaluated,and the evidence of the literature was extracted and summarized by two researchers.Results:A total of 12 articles were included,including 1 clinical decision,5 guidelines,2 expert consensus,2 systematic reviews and 2 evidence summaries.It summarized 6 topics,including follow-up personnel,follow-up methods,follow-up frequency,follow-up evaluation content,evaluation and improvement of self-management ability,and health education,with a total of 22 pieces of evidence.Conclusions:This study summarized the best evidence of discharge follow-up of AECOPD patients.Medical staff should make a comprehensive judgment based on the actual situation of patients when using evidence,take the best nursing measures,and carry out follow-up management of patients after discharge in a scientific way.
