内热针与传统针刺治疗脑卒中后上肢运动障碍的疗效对比分析

Comparative Analysis of Curative Effect of Internal Thermal Acupuncture and Traditional Acupuncture on Post-Stroke Upper Limb Dyskinesia

目的:探讨内热针与传统针刺治疗缺血性脑卒中后上肢运动障碍的疗效。方法:随机将2020年8月—2021年8月本院康复医学科收治的122例缺血性脑卒中后痉挛期上肢运动障碍患者分为对照组与观察组,各61例。对照组采用传统针刺治疗,观察组采用内热针治疗,均治疗2个疗程。观察上肢运动功能、手功能、上肢肌群共同收缩率(CR)、肩部疼痛程度、肌肉痉挛程度、生活质量和不良反应。结果:122例患者中110例完成治疗,每组各55例。两组治疗后FMA-UE评分、上肢Brunnstrom、BBT和MBI评分较治疗前增加,差异具有统计学意义(P<0.05),且观察组治疗后高于对照组,差异具有统计学意义(P<0.05)。两组治疗后屈曲位肱二头肌CR、伸展位肱三头肌CR、VAS评分与MAS评分较治疗前降低,差异具有统计学意义(P<0.05),且观察组治疗后评分低于对照组,差异具有统计学意义(P<0.05)。结论:与传统针刺比较,内热针可更有效地缓解患者肌肉痉挛和疼痛程度,改善上肢和手运动功能,提高患者生活质量,在缺血性脑卒中后上肢功能障碍治疗中具有更高的应用价值。

Objective:To investigate the effect of internal thermal acupuncture and traditional acupuncture on post-stroke upper limb dyskinesia.Methods:122 patients with post-stroke spasmodic upper limb dyskinesia were randomly divided into the control group and the observation group.The control group was treated with traditional acupuncture,whereas the observation group was treated with internal thermal acupuncture,both groups were treated for 2 courses.Upper limb motor function,hand function,common contraction rate(CR)of upper limb muscle groups,shoulder pain,muscle spasm,quality of life and adverse reactions were observed in the two groups.Results:110 of the 122 patients completed the treatment,with 55 in each group.After the treatment,FMA-UE score,Brunnstrom score of upper limbs,BBT score and MBI score were increased compared with those before the treatment in the two groups(P<0.05),of which the scores were higher in the observation group than those in the control group(P<0.05).CR biceps in flexion position and extension position,VAS score and MAS score were lower after the treatment than those before the treatment in the two groups(P<0.05),which were lower in the observation group than those in the control group after the treatment(P<0.05).Conclusion:Compared with traditional acupuncture,internal thermal acupuncture can relieve muscle spasm and pain more effectively,improve motor function of upper limbs and hands,improve life quality of the patients,which has a higher application value in treatment of post-stroke upper limb dyskinesia.
