非高密度脂蛋白胆固醇、甘油三酯与高密度脂蛋白胆固醇比值及血清甘油三酯葡萄糖乘积指数对40岁以上女性新诊断糖尿病前期患病的预测价值 被引量:7

The value of non-high-density lipoprotein cholesterol,ratio of triglyceride to high-density lipoprotein cholesterol and triglyceride-glucose index in predicting new prediabetes in women over 40 years old

摘要 目的探讨非HDL-C(non-HDL-C)、TG与HDL-C比值(TG/HDL-C)和血清甘油三酯葡萄糖乘积(TyG)指数对40岁以上女性新诊断糖尿病前期患病的预测价值。方法选取2011年8~12月大连地区40岁以上女性社区居民2725名,OGTT结果正常,随访3年后最终纳入2093名。受试者根据non-HDL-C四分位间距分为N1、N2、N3、N4组,根据TG/HDL-C四分位间距分为T1、T2、T3、T4组,根据TyG指数四分位间距分为Q1、Q2、Q3、Q4组。分析non-HDL-C、TG/HDL-C及TyG指数对糖尿病前期患病的预测价值。结果各组糖尿病前期患病率逐渐升高(P<0.05)。二元Logistic回归分析显示,TG/HDL-C、TyG指数是糖尿病前期患病的影响因素。受试者工作特征(ROC)曲线分析显示,non-HDL-C、TG/HDL-C、TyG指数预测糖尿病前期患病的ROC曲线下面积为0.560、0.639及0.669,最佳切点为4.055 mmol/L、1.137及6.938。结论TG/HDL-C、TyG指数是40岁以上女性糖尿病前期患病的影响因素,non-HDL-C、TG/HDL-C、TyG指数可预测糖尿病前期患病。 Objective To investigate the predictive value of non-high-density lipoprotein cholesterol(non-HDL-C),ratio of triglyceride to high-density lipoprotein cholesterol(TG/HDL-C)and triglycerideglucose(TyG)index for new onset prediabetes in women over 40 years old.Methods 2725 female community residents over 40 years old in Dalian from August to December 2011 were selected.Their OGTT results were normal,and 2093 were finally included after 3 years of follow-up.Subjects were divided into N1,N2,N3 and N4 groups according to the level of non-HDL-C,T1,T2,T3 and T4 groups according to the level of TG/HDL-C,and Q1,Q2,Q3 and Q4 groups according to the quartile of TyG index.The predictive value of non HDL-C,TG/HDL-C and TyG index for prediabetes were analyzed.ResultsThe prevalence of prediabetes in each group gradually increased(P<0.05).Binary logistic regression analysis showed that TG/HDL-C and TyG index were the influencing factors of prediabetes.ROC analysis showed that the area under the curve was 0.560,0.639 and 0.669,and the best cutoff points were 4.055 mmol/L,1.137 and 6.938.Conclusion TG/HDL-C and TyG index are the influencing factors for the occurrence of prediabetes.The levels of non HDL-C,TG/HDL-C and TyG index can predict the prediabetes.
