面部血管重建结合手机AR显示技术在提高面部注射填充美容安全性的应用研究

Application of facial vascular reconstruction combined with mobile phone AR display technology in improving the cosmetic safety of facial injection filling

摘要 目的评估面部血管重建结合手机AR显示技术在提高面部注射填充美容安全性的应用效果。方法自2020年11月至2022年8月,宜宾市第二人民医院医疗美容科行面部注射填充的47例患者,随机分为观察组(24例)和对照组(23例)。观察组术前行面部血管CTA检查,将CTA原始影像以DICOM格式保存并导入软件3D Slicer,重建出个体化面部血管三维模型,导入手机增强现实技术(augmented reality,AR)软件,与患者面部配准后,根据叠加影像描记面部血管走行,避开标记进行注射治疗;对照组术者根据手术经验进行注射。统计两组手术时间,术后即刻疼痛评定,以及术后面部血肿、瘀斑、组织坏死和失明发生情况。结果观察组与对照组手术时间比较,差异无统计学意义(P>0.05),观察组术后即刻疼痛程度低于对照组(P<0.05),术后面部血肿、瘀斑发生次数均低于对照组(P<0.05),对照组发生1例组织坏死,两组患者均无严重并发症发生。结论在面部注射填充美容手术中,面部血管重建结合手机AR显示技术可有效避开注射区域血管,降低手术并发症,具有一定的临床推广价值。 Objective To evaluate the effect of facial vascular reconstruction combined with mobile phone AR display technology in improving the cosmetic safety of facial injection filling.Methods A prospective study was conducted on 47 cosmetic patients who underwent injection filling in the Department of Medical cosmetology of the Second People’s Hospital of Yibin from November 2020 to August 2022.All patients were randomly divided into experimental group(24 cases)and control group(23 cases).The original CTA images were saved in DICOM format and imported into the software 3D Slicer to reconstruct the personalized three-dimensional model of facial blood vessels.The original CTA images were imported into the AR software of mobile phone.After registration with the face of the patients,the facial blood vessels were traced according to the superimposed images,and the injection treatment was performed without labeling.The control group was injected according to the operation experience.The operation time,immediate postoperative pain assessment,and postoperative facial hematoma,ecchymosis,tissue necrosis and blindness were recorded.Results Compared with the control group,there was no difference in operation time in the experimental group(P>0.05),the degree of immediate postoperative pain in the experimental group was lower than that in the control group(P<0.05),and the frequency of facial hematoma and ecchymosis after operation was lower than that in the control group(P<0.05).There was 1 case of tissue necrosis in the control group,and no serious complications of blindness occurred in the two groups.Conclusion In facial injection and filling cosmetic surgery,facial vascular reconstruction combined with mobile phone AR display technology can effectively avoid the blood vessels in the injection area and reduce surgical complications,which has certain clinical application and promotion value.
