镜像A-T皮瓣在面部美学分区中的应用

Application of mirror A-T flap in facial cosmetic zonation

目的探讨面部缺损后在面部美学分区中采用镜像A-T皮瓣修复的临床效果。方法自2012年11月至2020年6月,南京中医药大学附属医院整形外科对21例患者的鼻翼沟、唇红缘处色素痣或瘢痕,切除后的缺损部分按照鼻翼沟、唇红缘等面部美学单位分界线分为两部分,行镜像A-T皮瓣修复缺损,观察修复效果,并进行VSS评分。结果皮瓣全部成活,切口一期愈合,面部美学单位分界处无条索状瘢痕增生,术后随访6个月,VSS评分均为1分,效果满意。结论应用镜像A-T皮瓣修复横跨面部美学单位缺损效果较好,操作简便,值得临床推广应用。

Objective To investigate the clinical effect of mirror A-T flap in facial cosmetic zonation.Methods From November 2012 to June 2020,21 patients had pigmented nevus or scar at alar sulcus or labial margin.After lesion excision,the skin defect was divided into two parts according to facial cosmetic zonation demarcation line such as alar sulcus or labial margin,and the defect was repaired with A-T flap respectively,observing the effect of repairing and perform VSS scoring.Results All flaps survived,the incision healed primarily,and there was no cord scar hyperplasia at the boundary of facial cosmetic zonation.After 6 months of postoperative follow-up,the VSS score was 1 point,and the effect was satisfactory.Conclusion The application of mirror A-T flap to repair the defect across the facial cosmetic zonation is reliable and simple,it is worthy of clinical promotion and application.
