远程监控提高早期居家腹膜透析患者手卫生依从性效果观察

Observation on application effect of remote monitoring on hand hygiene compliance of early home peritoneal dialysis patients

目的探讨远程监控管理提高居家腹膜透析患者手卫生依从性的应用效果。方法选取23例居家腹膜透析患者作为调查对象,先后采用传统随访管理模式和远程监控模式对患者在居家腹膜透析操作过程中的手卫生依从性进行干预。结果应用远程监控管理模式后,居家腹膜透析患者的手卫生依从率由干预前的40%上升至干预后的94.2%,差异有统计学意义(χ^(2)=203.30,P<0.01)。采用传统随访管理模式时,患者在腹膜透析操作前、连接外接短管前、盖碘伏帽前和操作后4个阶段手卫生依从率分别为100%、0%、0%和60%,腹膜透析相关腹膜炎感染率为4.3%;采用远程监控管理模式后,相应阶段的手卫生指征依从性分别为100%、89%、97%和92%,并且无腹膜透析相关性感染。结论远程监控管理模式对提高居家腹膜透析患者手卫生依从性具有显著效果。

Objective To explore the application effect of remote monitoring management on improving hand hygiene compliance of patients with peritoneal dialysis at home.Methods 23 patients with peritoneal dialysis at home were selected as the subjects of this study.Traditional follow-up management mode and remote monitoring mode were used to intervene the hand hygiene compliance of patients with peritoneal dialysis at home successively.Results After remote monitoring management mode,the hand hygiene compliance rate of home peritoneal patients increased from 40%before intervention to 94.2%after intervention,and the difference was statistically significant(χ^(2)=203.30,P<0.01).After traditional follow-up management,the compliance of four hand hygiene indicators before peritoneal dialysis,before connecting external short tube,before capping iodophor cap and after peritoneal dialysis was 100%,0%,0%and 60%,respectively,and the peritonitis infection rate was 4.3%.The compliance of four hand hygiene indicators before peritoneal dialysis operation,before connecting external short tube,before capping iodophor cap and after operation were 100%,89%,97%and 92%,respectively.There was no peritoneal dialysis-associated infection.Conclusion Remote monitoring management mode has significant effect on improving hand hygiene compliance of peritoneal dialysis patients.
