超声心动图多切面检测结合超声高清血流显像对胎儿双主动脉弓畸形的诊断价值 被引量:1

摘要 目的 探讨超声心动图多切面检测结合超声高清血流(HD-flow)显像在双主动脉弓畸形胎儿产前诊断中的诊断价值。方法 选取在厦门市妇幼保健院行产前超声检查的疑似双主动脉弓畸形胎儿50例为观察组,同时选取正常胎儿50例为对照组,两组孕妇均完善超声心动图多切面检测结合HD-flow显像检查,观察胎儿的超声显像,并分析两者对双主动脉弓畸形胎儿的诊断价值。结果 超声心动图多切面检测结果为双主动脉弓畸形患儿阳性率为84.00%,阴性率为16.00%,HD-flow显像结果为双主动脉弓畸形患儿阳性率为86.00%,阴性率为14.00%。超声心动图多切面检测诊断双主动脉弓畸形患儿灵敏度为0.807,特异度为0.843,准确度为0.820,HD-flow显像结果诊断双主动脉弓畸形患儿灵敏度为0.860,特异度为0.860,准确度为0.860,两者联合诊断双主动脉弓畸形患儿的灵敏度为0.900,特异度为0.840,准确度为0.870。结论 超声心动图多切面检测结合HD-flow显像能明显提高双主动脉弓畸形的诊断灵敏度和特异度,具有一定的临床价值。 Objective To investigate the diagnostic value of multi-section echocardiography combined with high-definition(HD) flow imaging in prenatal diagnosis of double aortic arch malformation.Methods Fifty cases of double aortic arch malformation fetuses who underwent prenatal ultrasound examination in Xiamen Municipal Maternal and Child Health Care Hospital were selected as observation group, and 50 cases of normal fetuses were selected as control group.Both groups of pregnant women improved the multi-section detection of echocardiography combined with HD flow imaging, the fetal ultrasound imaging was observed, and the diagnostic value of both for double aortic arch malformation fetuses was analyzed.Results The positive rate and negative rate of echocardiography were 84.00% and 16.00%,respectively.The positive rate and negative rate of HD flow imaging were 86.00% and 14.00%,respectively.The sensitivity of multi-section echocardiography in diagnosis of double aortic abnormalities was 0.807, the specificity was 0.843,the accuracy was 0.820,the sensitivity of HD flow imaging in diagnosis of double aortic abnormalities was 0.860, the specificity was 0.860,the accuracy was 0.860, the sensitivity of multi-section echocardiography combined with HD flow imaging in diagnosis of double aortic abnormalities was 0.900,the specificity was 0.840,the accuracy was 0.870.Conclusion Multi-section echocardiography combined with HD flow imaging can significantly improve the diagnostic sensitivity and specificity of double aortic abnormalities, which has a certain clinical value.
