达标理论对卒中后偏瘫患者体位管理的影响

Influence of postural management based on the theory of goal attainment on the post-strokehemiplegia

目的 探究达标理论的体位管理对卒中后偏瘫患者良肢位和自我效能的影响。方法 选择郑州大学第二附属医院神经内科2个病区卒中后偏瘫患者50例,随机选取一个病区25例患者为对照组,另一个病区的25例患者为观察组,对照组采用常规干预,观察组应用达标理论的体位管理。对比2组患者良肢位的摆放率、合格率、自我效能和护理满意度。结果 干预3周后第2天,观察组良肢位的摆放率97.3%,合格率93.2%,均高于对照组(分别为80%、73.3%,P<0.05)。观察组体位管理后自我效能和护理满意度各维度得分均高于本组干预前(P<0.05),且观察组自我效能和护理满意度各维度得分均优于对照组(P<0.05)。结论 对卒中后偏瘫患者应用达标理论的体位管理有助于提高患者良肢位摆放的质量,提高自我效能,对改善患者生存质量和预后有积极意义。

Objective To explore the effect of posture management based on the theory of goal attainment on good limb position and self-efficacy in patients with hemiplegia after stroke.Methods Fifty patients with hemi⁃plegia after stroke from two wards of Neurology Department of the Second Affiliated Hospital of Zhengzhou Univer⁃sity were selected.A total of 25 patients from one ward were randomly selected as the control group,and 25 pa⁃tients from another ward were selected as the observation group.The control group was given routine intervention,and the observation group was given postural management based on the theory of reaching the standard.The place⁃ment rate of good limb position,qualified rate,self-efficacy and nursing satisfaction of patients in the two groups were compared.Results On the second day after 3 weeks of intervention,the placement rate of good limb posi⁃tion and qualified rate in the observation group were 97.3%and 93.2%,which were higher than 80%and 73.3%in the control group(P<0.05).The scores of each dimension of self-efficacy and nursing satisfaction in the obser⁃vation group after position management were higher than those before intervention(P<0.05),and the scores of each dimension of self-efficacy and nursing satisfaction in the observation group were better than those in the con⁃trol group(P<0.05).Conclusion The application of posture management based on the theory of goal attainment in patients with hemiplegia after stroke can help improve the quality of good limb position and self-efficacy,which has positive significance for improving the quality of life and disease prognosis.
