益气活血汤治疗血液透析糖尿病肾病临床观察

Clinical Observation on Yiqi Huoxue Decoction in the Treatment of Hemodialysis with Diabetic Nephropathy

目的探究益气活血汤对糖尿病肾病行血液透析患者的疗效及血清炎性因子的影响。方法选取2020年1月—2021年1月中国医科大学附属第一医院糖尿病肾病血液透析患者86例,随机分为2组,各43例。对照组采用常规血液透析治疗,并口服厄贝沙坦片和利格列汀片;试验组联合益气养血汤治疗。比较2组患者临床疗效、肾功能及血清炎性因子水平。结果试验组治疗总有效率为97.67%(42/43),高于对照组的74.42%(32/43)(P<0.05);与对照组比较,试验组治疗后血肌酐(Scr)、24 h蛋白尿及蛋白排泄率(UAER)水平较低(P<0.05),血清T肿瘤坏死因子α(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、白细胞介素-1(IL-1)水平较低(P<0.05);试验组患者恶心呕吐、腹泻、皮下出血等不良反应的发生率为9.30%(4/43),低于对照组的27.91%(12/43)(P<0.05)。结论益气活血汤对糖尿病肾病血液透析患者的疗效显著,可提高肾功能,改善血清炎性因子水平,且降低透析过程不良反应低,值得临床推广。

Objective To explore the effect of Yiqi Huoxue decoction on hemodialysis patients with diabetic nephropathy and serum inflammatory factors.Methods 86 hemodialysis patients with diabetic nephropathy from January 2020 to January 2021 were randomly divided into two groups,in with 43 cases each group.The control group received routine hemodialysis,Irbesartan and Liggliptin treatment,and the experimental group received more with Yiqi Yangxue decoction.The clinical effect,renal function and serum inflammatory factors were compared between the two groups.Results The total effective rate of the treatment group was 97.67%(42/43),which was higher than 74.42%(32/43)of control group(P<0.05).Compared with the control group,the serum creatinine(Scr)of the experimental group after treatment was 24 h albuminuria and protein excretion rate(UAER)levels were lower(P<0.05),serum T tumor necrosis factor-α(TNF-α),hypersensitive C-reactive protein(hs-CRP),interleukin-6(IL-6),interleukin-1(IL-1)levels were lower(P<0.05).The incidence of adverse reactions such as nausea,vomiting,diarrhea and subcutaneous bleeding in experimental group was 9.30%(4/43),which was lower than that in control group(27.91%,12/43)(P<0.05).Conclusion Yiqi Huoxue decoction has significant effect on hemodialysis patients with diabetic nephropathy.It can improve renal function,improve serum inflammatory factors,and reduce adverse reactions during dialysis.It is worthy of clinical promotion.
