脊髓损伤后神经源性膀胱尿潴留给予汤剂并穴位敷贴的疗效分析

Analysis of the Efficacy of Giving Soup and Acupuncture Point Com⁃presses for Neurogenic Bladder Urinary Retention After Spinal Cord In⁃jury

目的 分析脊髓损伤后神经源性膀胱尿潴留给予汤剂并穴位敷贴的疗效。方法 于2020年5月—2022年4月漳州市联勤保障部队第九〇九医院收治的脊髓损伤后神经源性膀胱尿潴留患者中方便选取124例作为研究对象,并随机分入对照组(盆底肌电刺激治疗)和观察组(盆底肌电刺激治疗+汤剂并穴位敷贴治疗),每组62例。比较两组尿流动力学指标、排尿情况、治疗效果、尿路感染发生率。结果 与对照组相比,观察组治疗后膀胱内压、残余尿量、排尿次数均更低,逼尿肌压力、平均尿流速、最大尿流速、膀胱安全容量、平均排尿量、最大尿量均更高,差异有统计学意义(P<0.05);观察组治疗总有效率为93.55%高于对照组的80.65%,尿路感染发生率为4.84%低于对照组的19.35%,差异有统计学意义(χ^(2)=4.593、6.143,P<0.05)。结论 脊髓损伤后神经源性膀胱尿潴留给予汤剂并穴位敷贴的疗效显著,可有效改善尿流动力学指标和排尿情况,降低尿路感染的发生率。

Objective To analyze the efficacy of giving soup and acupuncture point dressing for neurogenic bladder urinary retention after spinal cord injury.Methods 124 patients with neurogenic bladder urinary retention after spinal cord injury admitted to the 909th Hospital of Fujian Province Zhangzhou Joint Security Force from May 2020 to April 2022 were conveniently selected as the study objects.And randomly divided into control group(pelvic floor muscle electrical stimulation treatment)and observation group(pelvic floor muscle electrical stimulation treatment+tonics and acupuncture point dressing treatment),62 cases in each group.The urodynamic index,urination,treatment effect,and incidence of urinary tract infection were compared between the two groups.Results Compared with the control group,the observation group had lower intravesical pressure,residual urine volume,and number of voiding,and higher forced urinary muscle pressure,mean urinary flow rate,maximum urinary flow rate,safe bladder capacity,mean urinary volume,and maximum urinary volume after treatment,the difference was statistically significant(P<0.05).The total efficiency of treatment in the observation group was 93.55%higher than that in the control group 80.65%,and the incidence of urinary tract infection was 4.84%lower than that in the control group 19.35%,the differ⁃ence was statistically significant(χ^(2)=4.593,6.143,P<0.05).Conclusion The efficacy of giving soup and acupuncture point dressing for urinary retention of neurogenic bladder after spinal cord injury is significant.It can effectively im⁃prove the urodynamic index and urination and reduce the incidence of urinary tract infection.
