切开复位内固定术治疗复杂踝关节骨折的临床效果分析

Clinical Effect Analysis of Complex Ankle Fractures Treated by Incisional Internal Fixation

目的 分析切开复位内固定术治疗复杂踝关节骨折的临床疗效。方法 方便选取2016年8月—2021年10月南京市红十字医院收治的复杂踝关节骨折患者74例为研究对象,随机分为对照组(37例,手法复位联合石膏外固定)和观察组(37例,切开复位内固定术)。对比两组临床疗效、治疗时间、踝关节功能评分、踝关节运动度数和生活质量。结果 观察组治疗有效率(94.59%)显著高于对照组(75.68%),差异有统计学意义(χ^(2)=5.232,P<0.05);观察组患者的住院、下地活动和骨折愈合时间均显著短于对照组,差异有统计学意义(P<0.05);治疗后,观察组患者的行走、跑步、疼痛、工作能力、踝关节稳定性的踝关节功能评分均显著高于对照组,差异有统计学意义(P<0.05);观察组患者的踝关节旋后、旋前、屈、伸的运动角度均显著大于对照组,差异有统计学意义(P<0.05);观察组患者各项WHOQOL-100量表评分均显著高于对照组,差异有统计学意义(P<0.05)。结论 切开复位内固定术治疗复杂踝关节骨折,可获得显著的临床疗效,加速患者的康复,促进踝关节功能恢复,从而改善生活质量。

Objective To analyze the clinical efficacy of incisional internal fixation for complex ankle fractures.Meth⁃ods 74 patients with complex ankle fractures admitted to Nanjing Red Cross Hospital from August 2016 to October 2021 were conveniently selected as the research object and randomly divided into a control group(37 cases,manual repositioning combined with external fixation in plaster)and an observation group(37 cases,incisional repositioning internal fixation).The clinical efficacy,treatment time,ankle function score,ankle joint motion and quality of life were compared between the two groups.Results The treatment efficiency of patients in the observation group(94.59%)was significantly higher than that of the control group(75.68%),the difference was statistically significant(χ^(2)=5.232,P<0.05);the hospitalization,down time and fracture healing time of patients in the observation group were significantly shorter than that of the control group,the difference was statistically significant(P<0.05).After treatment,the ankle function scores of walking,running,pain,working ability and ankle stability of patients in the observation group were significantly higher than those in the control group,the difference was statistically significant(P<0.05);the motion angles of the ankle joint of posterior rotation,anterior rotation,flexion and extension of patients in the observation group were significantly higher than those in the control group,the difference was statistically significant(P<0.05);the WHOQOL-100 scale scores of patients in the observation group were significantly higher than those in the control group,the difference was statistically significant(P<0.05).Conclusion The treatment of complex ankle fractures with incisional internal fixation can achieve significant clinical efficacy,accelerate the rehabilitation of patients,promote the recovery of ankle function,and thus improve the quality of life.
