对比常规骨瓣开颅术、标准大骨瓣减压术应用于重型颅脑损伤患者治疗中的临床效果

Clinical Effect of Standard Large Flap Decompression,Conventional Flap Craniotomy in the Treatment of Patients with Heavy Craniocerebral In⁃jury

目的 研究重型颅脑损伤治疗中,常规骨瓣开颅术、标准大骨瓣减压术的治疗效果。方法 方便选取2017年1月—2020年1月宜兴市第二人民医院收治的65例重型颅脑损伤患者为研究对象,按照单双号分为两组。对照组(32例)实施常规骨瓣开颅术,研究组(33例)实施标准大骨瓣减压术,比较两组的疗效。结果手术后,研究组神经系统功能、意识炎性因子、脑血流动力学指标优于对照组,差异有统计学意义(P<0.05);研究组并发症发生率(6.06%)低于对照组(28.12%),差异有统计学意义(χ^(2)=5.625,P<0.05);研究组术后意识恢复时间、住院时间短于对照组,差异有统计学意义(P<0.05);研究组病死率(0.00%)低于对照组(18.75%),差异有统计学意义(χ^(2)=4.762,P<0.05)。结论 重型颅脑损伤的治疗,以标准大骨瓣减压术临床疗效较为良好,实施后能够有效恢复患者神经系统功能,维护其生命安全,术后恢复情况较为理想,疗效极佳。

Objective To study the treatment effect of conventional bone flap craniotomy,standard large bone flap de⁃compression in the treatment of heavy craniocerebral injury.Methods 65 patients with severe craniocerebral injury ad⁃mitted to the Second People´s Hospital of Yixing City from January 2017 to January 2020 were conveniently selected as the research object,and divided into two groups according to the distribution of single and double numbers.The control group(32 cases)underwent routine craniotomy with bone flap,and the study group(33 cases)underwent stan⁃dard large bone flap decompression to compare the efficacy of the two groups.Results After the operation,the nervous system function,consciousness,inflammatory factors and cerebral hemodynamic indexes in the study group were bet⁃ter than those in the control group,the difference was statistically significant(P<0.05);the incidence of complications in the study group was 6.06%lower than that in the control group(28.12%),the difference was statistically significant(χ^(2)=5.625,P<0.05);the time of postoperative consciousness recovery and hospital stay in the study group were shorter than those in the control group,the difference was statistically significant(P<0.05);the mortality rate in the study group was 0.00%lower than that in the control group(18.75%),the difference was statistically significant(χ^(2)=4.762,P<0.05).Conclusion For the treatment of severe craniocerebral injury,the clinical efficacy of standard large bone flap decompression is better,and the implementation can effectively restore the neurological function of patients and main⁃tain their life safety,and the postoperative recovery is more ideal with excellent efficacy.
