腹腔镜卵巢囊肿剥脱术与孕三烯酮治疗卵巢巧克力囊肿患者的临床效果及对卵巢功能的影响

Clinical Effect of Laparoscopic Ovariectomy and Gestational Trienone in the Treatment of Chocolate Cyst of Ovary and the Effect of Ovarian Func⁃tion

目的 分析应用孕三烯酮与腹腔镜卵巢囊肿剥脱术联合治疗卵巢巧克力囊肿的临床效果及对患者卵巢功能的影响。方法 回顾性选取2020年3月—2021年3月期间因卵巢巧克力囊肿进入中国人民解放军联勤保障部队第九〇〇医院治疗的120例患者。依照治疗方案进行分组,将接受腹腔镜卵巢巧克力囊肿剥脱术治疗的59例患者作为对照组,将接受孕三烯酮与腹腔镜卵巢囊肿剥脱术联合治疗的61例患者作为观察组。比较两组不良反应(便秘、头晕、恶心呕吐、头痛)发生情况及治疗前后窦状卵泡数量、雌二醇、黄体生成素、卵泡促激素及卵泡黄素化、卵泡发育萎缩、无卵泡发育情况。结果 治疗后,观察组窦状卵泡数量较对照组少,雌二醇、黄体生成素、卵泡促激素较对照组低,差异有统计学意义(P<0.05);治疗后,对照组与观察组的卵泡黄素化率分别为25.42%、3.28%,卵泡发育萎缩率分别为15.25%、0.00%,无卵泡发育率分别为16.95%、1.64%,差异有统计学意义(χ^(2)=12.096、7.981、8443,P<0.05);观察组不良反应发生率为11.48%,与对照组的15.25%比较,差异无统计学意义(P>0.05)。结论 应用孕三烯酮与腹腔镜卵巢囊肿剥脱术联合治疗卵巢巧克力囊肿的安全性好,能够调整性激素水平,有助于改善患者的卵巢功能。

Objective To analyze the clinical effect of pregntrienone combined with laparoscopic ovarian cyst exfolia⁃tion in the treatment of chocolate ovarian cyst and the effect on ovarian function.Methods A total of 120 patients with chocolate cyst of ovary admitted to the 900th Hospital of the Chinese People´s Liberation Army Joint Logistic Support Force from March 2020 to March 2021 were retrospectively selected.According to the treatment plan,59 patients who received laparoscopic chocolate ovarian cyst abruption as control group,and 61 patients who received combined treat⁃ment with pregntrienone and laparoscopic ovarian cyst abruption as observation group.The occurrence of adverse reac⁃tions(constipation,dizziness,nausea and vomiting,headache)and the number of sinusoidal follicles,estradiol,lutein⁃izing hormone,follicle-stimulating hormone,follicular luteinization,follicular atrophy and no follicular development before and after treatment were compared between the two groups.Results After treatment,the number of sinusoidal follicles in the observation group was less than that in the control group,and the estradiol,luteinizing hormone and follicle-stimulating hormone were lower than those in the control group,the difference was statistically significant(P<0.05).After treatment,the follicular luteinization rates of the control group and the observation group were 25.42%and 3.28%,the follicular atrophy rates were 15.25%and 0.00%,and the non-follicular development rates were 16.95%and 1.64%,respectively,and the difference was statistically significant(χ^(2)=12.096,7.981,8.443,P<0.05).The incidence of adverse reactions was 11.48%in the observation group,compared the 15.25%in the control group,the difference was not statistical significance(P>0.05).Conclusion The combination of gestational trienone and lapa⁃roscopic ovarian cyst stripping in the treatment of ovarian chocolate cyst has good safety,can adjust the sex hormone level,and help to improve the ovarian function of patients.
