老年AECOPD患者痰培养结果与外周血单个核细胞miR-125b、miR-133、miR-146a的相关性研究 被引量:1

摘要 目的观察老年慢性阻塞性肺疾病急性加重期(AECOPD)患者痰培养结果与外周血单个核细胞(PBMC)中微小RNA(miR)-125b、miR-133、miR-146a的相关性。方法回顾性选取2021年1月至12月在江苏东南大学附属南京同仁医院收治的79例老年AECOPD患者为AECOPD组,根据痰培养结果分为阳性组(n=39)和阴性组(n=40);另选取同期在该院收治的43例老年COPD稳定期患者作为COPD稳定组。检测PBMC中miR-125b、miR-133、miR-146a相对表达量与血清中炎症相关指标[降钙素原(PCT)、肿瘤坏死因子α(TNF-α)、白细胞介素-6(IL-6)]水平,分析各组间指标差异,并采用Spearman相关性分析AECOPD患者PBMC中miR-125b、miR-133、miR-146a与血清炎症指标的相关性。结果AECOPD组患者PBMC中miR-125b相对表达量、血清PCT、TNF-α、IL-6水平为0.87、0.38 ng/mL、41.98μg/L、250.01 pg/mL,高于COPD稳定组(0.49、0.05 ng/mL、21.16μg/L、83.63 pg/mL),miR-133、miR-146a相对表达量为0.57、0.56低于COPD稳定组(0.86、0.89),差异均有统计学意义(P<0.05)。AECOPD阳性组患者PBMC中miR-125b相对表达量、血清PCT、TNF-α、IL-6水平为0.94、0.42 ng/mL、50.38μg/L、273.93 pg/mL,水平高于阴性组(0.82、0.14 ng/mL、38.22μg/L、178.18 pg/mL),miR-133、miR-146a相对表达量0.55、0.40,低于阴性组(0.80、0.78),差异均有统计学意义(P<0.05)。PBMC中miR-125b水平与PCT、TNF-α、IL-6水平呈正相关(r=0.349、0.502、0.329,P<0.05),miR-133、miR-146a水平与PCT、TNF-α、IL-6水平呈负相关(r=-0.368、-0.411、-0.352;r=-0.389、-0.425、-0.374,P<0.05)。结论老年AECOPD痰培养阳性患者miR-125b呈现高表达,miR-133、miR-146a呈现低表达。 Objective To observe the correlation between sputum culture results and microRNA(miR)-125b,miR-133 and miR-146a in peripheral blood mononuclear cells(PBMC)in elderly patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 79 elderly patients with AECOPD admitted to the Nanjing Tongren Hospital Affiliated to Medical College of Southeast University were enrolled as AECOPD group between January and December 2021.According to the results of sputum culture,they were divided into positive group(n=39)and negative group(n=40).A total of 43 elderly patients with stable COPD during the same period were enrolled as stable COPD group.The relative expression levels of miR-125b,miR-133 and miR-146a in PBMC,and levels of serum inflammation-related indexes[procalcitonin(PCT),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)]were detected,and the differences of different indexes in different groups were analyzed.The correlation between miR-125b,miR-133,miR-146a and serum inflammatory indexes was analyzed by Spearman correlation analysis.Results The relative expression of miR-125b,and levels of serum PCT,TNF-αand IL-6 in PBMC of patients in AECOPD group were 0.87,0.38 ng/mL,41.98μg/L,250.01 pg/mL,which were higher than those in the stable COPD group(0.49,0.05 ng/mL,21.16μg/L,83.63 pg/mL),and the relative expression of miR-133 and miR-146a were 0.57 and 0.56,which were lower than those in the stable COPD group(0.86,0.89),the differences were statistically significant(P<0.05).The relative expression of miR-125b,and levels of serum PCT,TNF-αand IL-6 in PBMC of patients in positive group were 0.94,0.42 ng/mL,50.38μg/L,273.93 pg/mL,which were higher than those in the negative group(0.82,0.14 ng/mL,38.22μg/L,178.18 pg/mL),and the relative expression of miR-133 and miR-146a were 0.55,0.40,which were lower than those in the negative group(0.80,0.78),the differences were statistically significant(P<0.05).The relative expression level of miR-125b in PBMC was positively correlated with levels of PCT,TNF-αand IL-6(r=0.349,0.502,0.329,P<0.05),while levels of miR-133 and miR-146a were negatively correlated with them(r=-0.368,-0.411,-0.352;r=-0.389,-0.425,-0.374,P<0.05).Conclusion There is high-expression miR-125b,low-expression miR-133 and miR-146a in elderly AECOPD patients with positive sputum culture.
