多发性骨髓瘤患者FISH检测结果与Durie-Salmon分期和R-ISS分期的相关性分析 被引量:5

摘要 目的探究多发性骨髓瘤(MM)患者原位荧光杂交技术(FISH)检测结果与Durie-Salmon分期(DS)和R-ISS分期的相关性。方法回顾性选取2020年1月至2022年2月河北北方学院附属第一医院90例MM患者作为研究对象,所有患者均行FISH检测,主要包括lq21扩增、l3q14缺失、IGH重排与P53缺失,并予以DS分期与R-ISS分期,记录患者的临床特征,分析FISH检测结果与DS分期和R-ISS分期的相关性。结果FISH检测结果显示,90例MM患者中lq21扩增阳性35例,l3q14缺失阳性27例,IGH重排阳性24例,P53缺失4例;lq21扩增阳性与阴性患者在DS分期、R-ISS分期方面差异有统计学意义(P<0.05);l3q14缺失阳性与阴性患者在DS分期、R-ISS分期方面差异有统计学意义(P<0.05);IGH重排阳性与阴性患者在DS分期、R-ISS分期方面差异有统计学意义(P<0.05);P53缺失阳性与阴性患者在DS分期、R-ISS分期方面差异无统计学意义(P>0.05)。经过Spearson相关性分析,结果显示lq21扩增、l3q14缺失、IGH重排与DS分期和R-ISS分期均呈明显正相关(P<0.05);P53缺失与DS分期和R-ISS分期无相关性(P>0.05)。结论MM患者多伴随FISH检测结果异常,表现为lq21扩增、l3q14缺失、IGH重排与P53缺失,其中lq21扩增、l3q14缺失、IGH重排与患者DS分期、R-ISS分期呈正相关,可将FISH检测与Durie-Salmon分期、R-ISS分期联合应用于MM预后评估。 Objective To explore the correlation between the results of fluorescence in situ hybridization(FISH)with Durie-Salmon staging(DS)and R-ISS staging in patients with multiple myeloma(MM).Methods A total of 90 MM patients in the First Affiliated Hospital of Hebei North University from January 2020 to February 2022 were retrospectively selected as the research objects.All patients underwent FISH detection,including LQ21 amplification,L3Q14 deletion,IGH rearrangement and P53 deletion,and were divided into DS stage and R-ISS stage.The clinical characteristics of the patients were recorded.The correlation between FISH results and DS stage and R-ISS stage was analyzed.Results Among 90 MM patients,35 cases were LQ21 amplification positive,27 cases were L3Q14 deletion positive,24 cases were IGH rearrangement positive,and 4 cases were P53 deletion.There were significant differences in DS stage and R-ISS stage between LQ21 amplification positive and negative patients(P<0.05).There were significant differences in DS stage and R-ISS stage between L3Q14 deletion positive and negative patients(P<0.05).There were significant differences in DS stage and R-ISS stage between IGH rearrangement positive and negative patients(P<0.05).There was no significant difference in DS stage and R-ISS stage between P53 deletion positive and negative patients(P>0.05).Spearson correlation analysis showed that LQ21 amplification,L3Q14 deletion and IGH rearrangement were positively correlated with DS stage and R-ISS stage(P<0.05).There was no correlation between P53 deletion and DS stage and R-ISS stage(P>0.05).Conclusion MM patients are often accompanied by abnormal FISH test results,which are characterized by LQ21 amplification,L3Q14 deletion,IGH rearrangement and P53 deletion.Lq21 amplification,L3Q14 deletion,IGH rearrangement are positively correlated with DS stage and R-ISS stage.FISH,Durie-Salmon staging and R-ISS staging can be combined to evaluate the prognosis of MM.
