16排螺旋CT在脑梗死患者诊断中的效果

摘要 目的探究16排螺旋CT在脑梗死的诊断效果。方法在2019年1至9月中选择40例疑似脑梗死患者展开回顾性分析,均接受头颅CT平扫和脑CT灌注成像操作。结果发病时间均在24 h内,有20例患者被诊断为急性脑梗死,其余20例并未见异常;而后在1周内对20例未见异常患者通过CT复查和随访,证实其中急性脑梗死发作16例和短暂性脑缺血发作4例。36例患者脑CT灌注异常,4例脑CT灌注正常,并且感兴趣区内脑血流量、脑血容量以及对比剂达峰时间均出现显著转变,病灶侧较之对照侧、病灶周边区、病灶中心区均差异有统计学意义(P<0.05);且脑CT灌注成像灵敏度以及特异度均高于CT平扫。结论应用16层螺旋CT在脑梗死的临床诊断中具有较高的诊断价值,为临床医师诊断疾病、制定治疗方案和预估预后提供重要的参考依据。 Objective To analyze and explore the diagnostic effect of 16 slice spiral CT in cerebral infarction.Methods From January 2019 to September 2021,40 patients with suspected cerebral infarction were selected for retrospective analysis.They all underwent plain CT scanning and brain CT perfusion imaging.Results The onset time of all patients was within 24 hours.Twenty patients were diagnosed as acute cerebral infarction,and the other 20 cases were normal.Then,20 patients without abnormalities were confirmed by CT reexamination and follow-up within one week,in which the proportion of acute cerebral infarction and transient ischemic attack was 16:4 respectively.Thirty-six patients had abnormal cerebral CT perfusion,four patients had normal cerebral CT perfusion,and there were significant changes in cerebral blood flow,cerebral blood volume and peak time of contrast medium in the area of interest.There were significant differences in the focus side compared with the control side,the surrounding area and the central area of the focus(P<0.05).The sensitivity and specificity of brain CT perfusion imaging were higher than those of plain CT scanning.Conclusion 16 slice spiral CT has high diagnostic value in the clinical diagnosis of cerebral infarction.It provides an important reference for clinicians to diagnose the disease,formulate treatment plan and predict the prognosis.
