全髋关节置换术中运用FPP导向器指导安放髋臼前倾角外展角的临床研究

Clinical Study of Using FPP Guide to Guide Placement of Acetabular Ante⁃version Abduction Angle in Total Hip Arthroplasty

目的 探讨全髋关节置换术患者运用FPP导向器指导安放髋臼前倾角、外展角的效果。方法 随机选取2021年1—12月在汕尾市第二人民医院实施全髋关节置换术治疗的60例患者为研究对象,采用随机摸球法分为A组(n=30)与B组(n=30)。B组使用传统方法设定髋臼假体前倾角、外展角,A组使用FPP导向器指导安放髋臼前倾角、外展角,比较两组患者手术相关指标、髋臼假体前倾角、外展角、疼痛程度及髋关节功能。结果 A组手术时间(92.34±5.48)min较B组(104.51±5.87)min明显更短,差异有统计学意义(t=8.301,P<0.05);A组术中出血量(185.42±6.96)mL较B组(219.31±7.34)mL明显更少,差异有统计学意义(t=18.351,P<0.05);两组术后前倾角、外展角明显改善,A组较B组明显更优,差异有统计学意义(P<0.05);两组术后NRS评分明显降低,A组评分较B组明显更低,差异有统计学意义(P<0.05);两组术后髋关节评分量表(Harris)评分明显提高,A组评分较B组明显更高,差异有统计学意义(P<0.05)。结论 全髋关节置换术患者运用FPP导向器指导安放髋臼前倾角、外展角,能够改善患者手术相关指标、髋臼假体前倾角、外展角及髋关节功能,降低疼痛程度。

Objective To investigate the effect of using FPP guides to guide the placement of acetabular anteversion and abduction angles in patients undergoing total hip arthroplasty.Methods 60 patients who underwent total hip ar⁃throplasty in Shanwei Second People's Hospital from January to December 2021 were randomly selected for the study,and they were divided into group A(n=30)and group B(n=30)using the randomized ball touch method.Group B used the traditional method to set the anteversion and abduction angles of the acetabular prosthesis,while group A used the FPP guide to guide the placement of the anteversion and abduction angles of the acetabulum.The surgical related in⁃dexes,acetabular prosthesis anteversion and abduction angles,pain level and hip function were compared between the two groups.Results The operative time in group A(92.34±5.48)min was significantly shorter than that in group B(104.51±5.87)min,and the difference was statistically significant(t=8.301,P<0.05).Intraoperative bleeding in the group A(185.42±6.96)mL was significantly less than group B(219.31±7.34)mL,the difference was statistically sig⁃nificant(t=18.351,P<0.05).Postoperative anteversion and abduction angles improved significantly in two groups,and group A was significantly better than group B,the difference was statistically significant(P<0.05);postoperative numerical rating scale(NRS)scores were significantly lower in two groups,and group A significantly lower than group B,the difference was statistically significant(P<0.05).The postoperative Hip Rating Scale(Harris)scores were signifi⁃cantly higher in two groups,and the scores in group A were significantly higher than group B,the difference was statis⁃tically significant(P<0.05).Conclusion The use of the FPP guide to guide the placement of the acetabular anteversion and abduction angles in total hip arthroplasty patients can improve the surgery-related indexes,acetabular prosthesis anteversion and abduction angles and hip function,and reduce the degree of pain.
