PICC置管术后并发机械性静脉炎的多因素分析及其护理对策

Multivariate analysis of mechanical phlebitis complicating PICC catheterization and its nursing countermeasures

目的分析PICC置管术后并发机械性静脉炎的因素以及针对影响因素制定对应的护理干预策略。方法选取2016年1月—2019年5月在本院进行PICC置管术的患者235例作为研究对象,观察纳入患者术后是否并发机械性静脉炎,采用单因素分析及多因素Logistic回归分析筛选PICC置管术后患者并发机械性静脉炎独立危险因素,根据分析结果制定相应护理对策。结果单因素分析结果显示,性别、年龄、穿刺次数、置管时间、导管位移、导管位置、高血压、高血脂、糖尿病、运动刺激、SAS评分以及SDS评分与PICC置管术后并发机械性静脉炎有关(P<0.05);多因素Logistic回归分析发现,穿刺次数、置管时间、导管位移、导管位置、运动刺激为PICC置管术后并发机械性静脉炎的独立危险性因素(P<0.05)。结论穿刺次数、置管时间、导管位移、导管位置、运动刺激为PICC置管术后并发机械性静脉炎的独立危险性因素,医护人员应针对影响因素提供有效的干预,降低PICC置管术后机械性静脉炎发生率。

Objective To analyze the factors that complicated mechanical phlebitis after PICC catheterization and to develop corresponding nursing intervention strategies for influencing factors.Methods A total of 235 patients who underwent PICC catheterization in Zhongshan Affiliated Hospital from January 2016 to May 2019 were selected as study subjects to observe whether the included patients had postoperative mechanical phlebitis,and the independent risk factors of concurrent mechanical phlebitis in patients after PICC catheterization were screened by univariate analysis and multivariate logistic regression analysis,and the corresponding nursing countermeasures were developed based on the analysis results.Results In univariate analysis,sex,age,number of punctures,catheterization time,catheter displacement,catheter position,hypertension,hyperlipidemia,diabetes mellitus,exercise stimulus,SAS score,and SDS score were associated with the postoperative mechanical phlebitis after PICC catheterization(P<0.05);mulitivariate logistic regression analysis identified the number of punctures,catheterization time,catheter displacement,catheter position,and exercise stimulus as independent risk factors for the development of PICC catheterization complicated by mechanical phlebitis(P<0.05).Conclusion The number of punctures,catheterization time,catheter displacement,catheter position,and exercise stimulus provide insight into whether there is a close relationship between the occurrence of complicated mechanical phlebitis after PICC catheterization,and caregivers should provide effective interventions targeting influencing factors to reduce the incidence of mechanical phlebitis after PICC catheterization.
