2020~2021年蓟州区人民医院住院患者人血白蛋白临床应用情况分析

Analysis of clinical application of human serum albumin in inpatients in Jizhou District People's Hospital

目的调查分析蓟州区人民医院住院患者人血白蛋白的临床应用情况,为临床合理使用提供参考。方法采用回顾性分析方法,利用HIS系统抽取2020~2021年应用人血白蛋白住院患者临床资料,采用Excel软件对患者年龄、科室用药情况分布、使用剂量、用药疗程、用药原因以及用药前血清白蛋白水平进行统计分析,并根据药品说明书及相关指南对其合理性进行评价。结果3606例应用人血白蛋白患者主要涉及21个科室,主要以呼吸消化科、ICU、肝胆外科居多;个人平均用量为(65.38±84.20)g,平均用药疗程为(6.64±6.74)d,个人平均用量与平均用药疗程最多的均在ICU,用药前血清白蛋白水平多低于30 g/L。随机点评370份病例,用药原因以防治低蛋白血症、营养支持为主(达70%)。结论人血白蛋白的使用较广泛,在临床应用中仍存在不合理使用现象,进一步加强医师培训,严格掌握HSA使用指征,完善审批制度,成立专项点评小组,建立合理的用药评估体系,加大行政干预处罚力度,以促进用药规范。

Objective To investigate and analyze the clinical application of human serum albumin in inpatients in Jizhou District People's Hospital,and to provide reference for clinical rational use.Methods The retrospective analysis method was used,and the HIS system was used to extract the clinical data of inpatients with human serum albumin in our hospital from 2020 to 2021,and the Excel software was used to analyze the age of the patients,the distribution of drug use in the department,the dose used,the course of treatment,the reason for the drug and the drug use.The pre-serum albumin level was statistically analyzed,and its rationality was evaluated according to the drug insert and related guidelines.Results The 3606 patients with human serum albumin mainly involved 21 departments,mainly in respiratory gastroenterology,ICU,and hepatobiliary surgery;the average individual dosage was(65.38±84.20)g,and the average treatment course was(6.64±6.74)d.The average dosage and the average course of treatment for individuals are both in the ICU,and the serum albumin level before treatment is mostly lower than 30 g/L.370 cases were randomly reviewed,and the reasons for medication were mainly the prevention and treatment of hypoalbuminemia and nutritional support(up to 70%).Conclusion Human serum albumin is widely used,and there is still irrational use in clinical application.We should further strengthen the training of physicians,strictly grasp the indications for HSA use,improve the approval system,set up a special review group,and establish a reasonable drug evaluation system,and increase administrative intervention and punishment to promote drug use standards.
