妊娠期糖尿病患者中目标导向护理的应用分析

Analysis of the Application of Goal-Oriented Care in Patients with Gesta⁃tional Diabetes Mellitus

目的 评价对妊娠期糖尿病患者开展目标导向护理的应用效果及价值。方法 回顾性分析2021年3月—2022年4月平潭综合实验区医院妇产科50例妊娠期糖尿病患者的临床资料,随机分为两组,其中23例纳入对照组给予常规护理,其余27例纳入观察组开展目标导向护理;将两组患者的自我管理行为、不良妊娠结局以及护理前后的糖代谢、脂代谢水平加以观察和对比。结果 实施护理工作后,与对照组比较,观察组自我管理行为评分均较高,差异有统计学意义(t=2.292、4.337、2.309、5.531,P<0.05)。观察组患者糖代谢水平较对照组低,差异有统计学意义(P<0.05)。观察组患者TG、TC明显较对照组低,HDL-C明显较对照组高,差异有统计学意义(P<0.05)。两组LDL-C水平比较,差异无统计学意义(P>0.05)。观察组不良妊娠结局发生率(3.70%)较对照组(30.43%)低,差异有统计学意义(χ^(2)=4.764,P<0.05)。结论 对妊娠期糖尿病患者开展目标导向护理具有显著优势,可改善患者糖脂代谢水平并减少不良妊娠结局的发生。

Objective To evaluate the effect and value of applying goal-oriented care to patients with gestational dia⁃betes mellitus.Methods Retrospective analysis was performed on the clinical data of 50 patients with gestational diabe⁃tes in the Department of Obstetrics and Gynecology of Pingtan Comprehensive Experimental Area Hospital from March 2021 to April 2022.The patients were randomly divided into two groups,of which 23 were included in the control group for routine nursing,and the other 27 were included in the observation group for goal-oriented nursing.The self-management behavior,adverse pregnancy outcome,glucose metabolism and lipid metabolism before and after nursing were observed and compared between the two groups.Results After the implementation of nursing work,the self-management behavior scores of the observation group were higher than those of the control group,and the difference was statistically significant(t=2.292,4.337,2.309,5.531,P<0.05).Compared with the control group,the glucose me⁃tabolism level in the observation group was lower,and the difference was statistically significant(P<0.05).TG and TC in observation group were significantly lower than control group,and HDL-C was significantly higher than control group,the difference was statistically significant(P<0.05).There was no significant difference in LDL-C level between the two groups(P>0.05).The incidence of adverse pregnancy outcomes in the observation group(3.70%)was lower than that in the control group(30.43%),and the difference was statistically significant(χ^(2)=4.764,P<0.05).Conclusion Target-oriented care for patients with gestational diabetes mellitus has significant advantages,which can improve the level of glucose and lipid metabolism of patients and reduce the occurrence of adverse pregnancy outcomes.
